PETITION FOR APPOINTMENT OF A GUlR DIAN.
TO THE HONORABLE P.V.MARINO,
PRESIDENT JUDGE OF THE SAID COURT:
The Petition of DONA LD LEVERS,parent and natural guardian of
SALLY LEVERS,a minor,respectfully represents:
1.That your petitioner is the father and natural guardian of
Sally Levers,a minor,who reside together at 1405 Allison Avenue,Washing
ton,Washington County,Pennsylvania.
2.That the said Sally Levers,a minor,was born on April 1,
1964 and is presently seven years of age.
3.That Sally Levers,a minor,was involved in an automobile
accident on July 1,1970.From that occurrence your petitioner filed a suit
in the Court of Common Pleas of Washington County at No.149 May Term
1971 against Kenneth T.Kimble,Defendant.
4.That prior to trial,the suit was settled and after bills,
costs and fees are paid,the above named minor will receive the sum of
$12,542.60.
5.That the settlement referred to hereinabove was approvedOfby
the Washington County Court and Mellon National Bank and Trust Company
was designated to hold the money in trust for the minor until her majority.
6.Your petitioner therefore desires that your Honorable Court
make the necessary Order to appoint Mellon National Bank and Trust Compan
as guardian of the Estate of Sally Levers,a minor.
7.The said Sally Levers,a minor,has no other estate or propert
in her name except the above mentioned money.
8.That the proposed guardian has no interest adverse to the said
minor and is not the fiduciary or an officer,or employee,or the corporate
fiduciary of any estate in which the said minor has an interest.
WHEREFORE,your petitioner prays your Honorable Court that
Mellon l'lational Bank and Trust Company be appointed as guardian of the
Estate of Sally Levers,a minor,so that it may have and receive the afore-
said money to be held in trust for the said minor until her majority.

PETITION FOR ALLOWANCE
TO THE HONORABLE,THE JUDGES OF SAID COURT:
~NO NOW,comes JUANITA LEVERS,Petitioner,through
her attorneys,ROSENBERG,SEWAK &PIZZI,and petitions the
Court,as follows:
1.Your Petitioner,Juanita Levers,is the parent
and natural guardian of the minor,Sally Levers.
2.The minor child was born on April 1,1964,and
is now 15 years of age.She resides with her mother,the
Petitioner herein,Juanita Levers,at 151 Ford Street,
Washington,Washi:Q:gton:County,Pennsylvania.
3.Suit was brought on behalf of Sally Levers in the
Court of Common Pleas of Washington County at -No.149 May Term,
1971,to recover damages sustained by the minor as a result of
an automobile accident which occurred on July 1,1970.
4.A settlement was made in the above suit in the
amount of Twelve Thousand Five Hundred Forty-two and 60/100
($12,542.60)Dollars,said settlement being approved by the
Court of Common Pleas of Washington County,Pennsylvania.
5.As a result of said Court approval,Twelve Thousand
Five Hundred Forty-two and 60/100 ($12,542.60)Dollars was
deposited in Mellon National Bank and Trust Company,with
instructions that the sum not be distributed until the minor
reached the age of majority or unless a Court Order is presented.
6.Your Petitioner,Juanita Levers,has expended the
following amounts for the care and maintenance of the minor,
Sally Levers:
a.Appointment with Dr.Salviti
on November 23,1979 for an
eye examination which resulted
in the purchase of glasses.
b.Appointment with Dr.Gayed on
November 23,1979 in which
dental x-rays were taken and
theminor's-teeth were cleaned
and 9 fillings.were made.Also,
oral surgery was prescribed.
