PETITION FOR APPOINTMENT OF GUARDIAN
TO THE HONORABLE P.VINCENT MARINO,PRESIDENT JUDGE OF SAID
ORPHANS'COURT DIVISION:
The Petition of Thomas M.Paterra,respectfully represents:
1.That his full name is Thomas M.Paterra,and that he
resides at 307 Isabella Avenue,in the Borough of North Charleroi,
Washington County,Pennsylvania,and that he will be seventeen (17)
years of age on December 29th,1971,and that his father,Frank F.
Paterra,a widower,died intestate on October 28th,1971,his mother,
Virginia C.Paterra,having predeceased his father on February 6th,1968.
2.That at the time of the death of the father of your
Petitioner,namely,Frank F.Paterra,your Petitioner was residing
as a family with his said father,together with his two brothers,
Frank J.Paterra,age 26,and John P.Paterra,age 20,at their
family residence at 307 Isabella Avenue,in the Borough of North
Charleroi,this County,and that attached hereto are consents of the
said brothers,Frank J.Paterra and John P.Paterra,and also the
consent of their older sister,Patricia Kolodychak,nee Paterra,of
the City of Bloomington,State of Indiana,for the appointment of a
Guardian of the Estate of your Petitioner,as well as a Guardian for
the person of your Petitioner,which is hereinafter prayed for.
3.That your Petitioner does not have any Guardian for his
estate nor for his person,and that in this instance it is necessary
that a Guardian be appointed for the estate of your Petitioner,as well
as for his person,for the reason that your Petitioner has come into

the ownership of certain cash proceeds from policies of life
insurance,being from General American Life Insurance Company of
St.Louis,Missouri,and Metropolitan Life Insurance Company,in the
total approximate amount of $10,000.00,and is payable by reason of
the death of the said Frank F.Paterra,father of your Petitioner,and
your Petitioner was named as one of four beneficiaries (his two
brothers and sister being co-beneficiaries)in both of the said
insurance policies,and the payment of said insurance proceeds are
being withheld by the said Insurance Companies pending the appointment
of a Guardian of the Estate of your Petitioner.
4.The proposed Guardian of the Estate and of the person of
your Petitioner is Frank J.Paterra,hereinbefore named brother of
your Petitioner,who is presently head of their household in the said
Borough of North Charleroi where they continue to reside as a family,
together with the other brother,John P.Paterra,also hereinbefore
named.
5.That the proposed Guardian does not have any interest
adverse to the minor,nor is the proposed Guardian a fiduciary or an
officer or an employee of the corporate fiduciary of an estate in which
the minor has an interest,nor the surety or an officer or an employee
of the corporate surety of such a fiduciary.
6.That both your Petitioner and his brother,Frank J.Paterra,
the proposed Guardian of the estate and of the person of your
Petitioner,are of the Roman Catholic Faith.
7.That no notice is required to be given to the United
States Veterans Administration,as the father of the Petitioner was
not a veteran of the armed services of the United States of America.
8.That the assets of the estate of your Petitioner,being
insurance proceeds as hereinbefore mentioned,arise as follows:
General American Life Insurance Company--$8,000.00;Metropolitan
Life Insurance Company--$2,000.OO.
9.That the said Frank J.Paterra has consented to act as

both the Guardian of the estate of your Petitioner and Guardian of
the person of your Petitioner,and his written consent is attached
hereto.
~~EREFORE,your Petitioner prays that Frank J.Paterra or some
other competent party by appointed Guardian of his estate and
Guardian of his person.
