PETITION FOR APPOINTMENT OF GUARDIAN FOR
THE ESTATE OF ALLAN MONTECALVO,a minor.
TO THE HONORABLE P.VINCENT MARINO,PRESIDENT JUDGE OF THE SAID
COURT:
1.The petitioners are the parents of Allan Montecalvo,
18 years of age,he having been born on January 1,1953,and they
reside at 1042 Fourth Street,Canonsburg,Washington County,
pennsylvania.
2.That it is necessary to appoint a Guardian for the
Estate of the said minor for the reason that he is to receive the
sum of $5,000.00 in the settlement of a Trespass action.
3.That the said minor has no Guardian of his Estate.
4.The name and address of the proposed Guardian of the
said minor's estate is Elizabeth Ardizzone,2322 West pike Street,
Houston,pennsylvania.
5.That the proposed Guardian has no interest adverse
to the minor and is not the fiduciary or officer or employee of
a corporate fiduciary of any estate in which the minor has an
interest.
6.The said minor is entitled to the sum of $5,000.00
which sum is due him as a compromise settlement in a Trespass
action in the Court of Common Pleas of Washington County,
pennsylvania,at No.ao5 Se~t Term,1971,A.D.The sum

Montecalvo,appearing in open Court makes choice of Elizabeth
Ardizzone and upon consideration thereof,Elizabeth Ardizzone
is hereby appointed Guardian of the Estate of the said Allan
Montecalvo~as prayed for.
AND IT IS FURTHER DIRECTED that the sum of $5,000.00
shall be paid to Elizabeth Ardizzone as Guardian of Allan Montecal 0,
to be deposited by her in the First National Bank &Trust Co.,
Washington,Pennsylvania,said sum to remain in said account and
not to be withdrawn therefrom during the minority of said minor
without prior Orphans Court approval,and the said First National
Bank &Trust Co.,Washington,pennsylvania,is hereby authorized
to pay over the said sum of $5,000.00 plus accrued interest
directly to the said minor upon proper proof that the said minor
has attained his majority.

TO THE HONORABLE,THE JUDGES OF SAID COURT:
The petibion of ELIZABETH ARDIZZONE,Guardian of the
Estate of Allan Montecalvo,a minor,respectfully represents:

1.That your peti~ioner was dUly appointed guardian of

the Estate of Allan Montecalvo,a minor,by Decree of your
Honorable Court,dated the 20th day of September,1971,at No.
948 of 1971 O.C.
2.That the said Allan Montecalvo is twenty (20)years
of age,having been born on January 3,1953,and that he resides
with his mother at 1042 Fourth Street,Canonsburg,Washington
County,pennsylvania.
3.That the estate of said minor consists of $5,300.97
in cash including interest.
4.That the said minor is presently employed at Best Oil
Garage at Donaldson's Crossroads,McMurray,pennsylvania and it
will be necessary for him to purchase an automobile for use in
traveling to his place of emp1o~ent,f~r'the ~um of $1,000.00.
5.That the said minor has ~,ust started _his job~',and has
no source of income other than this Trust Fund and his parent
is unable to pay the above stated amount.
6.That the said minor has turned this bill over to your
petitioner,and has requested that this sum b~paid out of the
Trust Fund.

7.That the said minor is twenty (20)years of age,and
has received notice of the intended presentation of this petition
and has joined in the prayer thereof.
WHEREFORE,your petitioner prays for leave and authora&y
to pay the amount of ONE THOUSAND and nO/100 ($1,000.00)DOLLARS,
which sum is to be applied towards the payment of an automobile
for transportation to the minoris place of emplo~ent.
