PETITION FOR SETTLEMENT
OF SMALL ESTATE
TO THE HONORABLE, P. V. MARINO, PRESIDENT JUDGE:
The petition of ANNE T. SEWCHOK, through her attorneys,
BLOOM, BLOOM, ROSENBERG & BWOM, respectfully represents:
1. That your petitioner is the ~idow and sole heir of Nick
Sewchok, deceased.
2. That the decedent, Nick Sewchok, died intestate on the 3rd
day of March, 1971; his last place of residence being Gastonville,
Washington County. Pennsylvania.
3. That your petitioner has not applied for nor been granted
Letters of Administration.
4. That the estate of the decedent amounts to less than /
Twenty-five Hundred ($2500. 00) Dollars and is composed of the following:
PERSONAL PROPERTY
Amount due from Department of Health,
Eduction and Welfare under the Federal
Coal Mine Health and Safety Act $ 229. 60
REAL ESTATE
None

5. That all of the after .. described bills of the said decedent
are paid:
Edward B. Garry Funeral Home ..•. $ 1135. 00
Queen of Heaven Cemetery . . . . . . 328. 00
Washington Hospital -last illness. . . 145.00
6. That your petitioner is entitled to the above sum as the
decedent resided with your petitioner and she has a $1500. 00 family exemp-
tion.
7. That this petition for settlement is made pursuant to the
provisions of Article II, Section 202 of the Fiduciaries Act of 1949, as
amended, effective November 10, 1959, decedent having died domiciled in
the Commonwealth of Pennsylvania, County of Washington, with an estate
consisting entirely of personal property of a gross value not exceeding Twent
. Five Hundred ($2500. 00) Dollars.
WHEREFORE, your petitioner prays your Honorable Court to
enter a Decree of Distribution approving the settlement of this estate as
hereinabove set forth. And she will ever pray.

DECREE OF DISTRIBUTION
AND NOW, this
the foregoing Petition for Settlement of a Small Estate
upon consideration and upon motion of ANNE T. SEWCHOK, through her
attorneys, Bloom, Bloom, Rosenberg & Bloom, it is ORDERED, ADJUDGED
and DECREED that the entire amount of the estate of the decedent Nick Sewcho ,
deceased, consisting of $229. 60 shall be paid to the petitioner Anne~;T. Sewcho ,
