Miller,who died intestate on December 12,1965,leaving to survive him as his sole heir-at-1awDeborah J.Miller,minor. 4.That your Petitioner has no Guardian for her estate. 5.That your Petitioner was the named beneficiary under two (2)insurance policies owned by Olen R.Miller,the same being Policy No.23 015 301 and Policy No.19 599 928 with the • Prudential Insurance Company of America. 6.Joan M.Miller was the Administratrix of the Estate of Olen R.Miller which was concluded by final decree and distribution schedule tendered by the Court of Common Pleas of Washington County,Orphans'Court Division,on January 28,1970, awarding the sum of $2,707.64 to the minor,Deborah J.Miller. 7.Your Petitioner avers and sets forth that the appoint- ment of a guardian of the Estate of Deborah J.Miller,minor,is required and necessary in order to control,acquire and conserve the assets of the minor which are set forth as follows: (a)Savings account at Charleroi $2,707.64 Federal Savings and Loan Associ- ation under award of the Court of Common Pleas of Washington County, Orphans'Court Division,on January 28,1970,together with accrued interest. (b)Insurance policy no.23 015 301 with 5,000.00 Prudential Insurance Company of America together with interest due thereon to which the minor is entitled by virtue of having been named beneficiary therein. (c)Insurance policy no.19 599 928 with 5,155.91 Prudential Insurance Company of America together with interest due thereon to which the minor is entitled by virtue of having been named beneficiary thereon. Total $12,863.55 8.Attached hereto and made a part hereof as Exhibit "A" is the consent of Joan M.Miller to the appointment of a Guardian in these proceedings.Also attached hereto is the acceptance of Pittsburgh National Bank as Guardian. -2- 9.That your Petitioner suggests to your Honorable Court that Pittsburgh National Bank be appointed Guardian for her estate;that the said Pittsburgh National Bank is not a Fiduciary of any estate nor is it the surety of any Fiduciary in any estate in which the minor may have an interest and,in addition thereto,the said Pittsburgh National Bank has no interest advers to your Petitioner. WHEREFORE,your Petitioner prays your Honorable Court to appoint Pittsburgh National Bank or some other fit corporate fiduciary as Guardian of her estate. -3- .. TO THE HONORABLE,THE JUDGES OF SAID COURT; .•The P~tition.of 'Deborah J.Miller respectfully represents: 1.Your Petitioner filed a Petition for Appointment of a Guardian of the Estate of a Minor before your Honorable Court on June 21, 1971,.and received an Order and Decree appointing the Pittsburgh National Bank and Trust Company,as guardian for the Estate of said minor. 2.Your Petitioner is presently age thirteen (13)having been born on June 8,1957,and resides together with her Mother,Joan M.Miller, at 177 State Street,Speers Hill,Charleroi,Washington County,Pennsylvania. 3.Your Petitioner avers and sets forth that the guardian, Pittsburgh National Bank and TrUst Company,Joan M.Miller,the natural mother and Deborah J.Miller,the minor,agree that no necessity exists for the continuation of the guardianship. 4.Your Petitioner further avers and sets forth that the assets listed in Paragraph 7 of the original petition,to-wit,the following assets: A.Savings Account at Charleroi Federal Savings and Loan Association under an award of the Court of Common Pleas of Washington County,Orphans I Court Division,on January 28,1970,together with accrued interest.
