for the Estate of
RANDY S. ANSEIMINO, a
minor

PETITION
The Petition of Patricia Anselmino, parent and natural guardian
of Randy S. Anselmino, a minor, respectfully represents as follows:
1. That Randy S. Anselmino resides with his parent at Main Street,
New Eagle, Washington County, Pennsylvania, and is eleven (11) years of
age, born September 18, 1959.
2. Your petitioner is the parent of the minor and resides at
Main Street, New Eagle, Washington County, Pennsylvania.
3. That the minor is in need of the appointment of a Guardian of
his estate since the minor is entitled to receive certain proceeds_from
the settlement of a damage claim.
4. The Western Pennsylvania National Bank, Pittsburgh, Pennsylvania
is the proposed Guardian of the estate of the minor and their consent is
hereto attached and made a part of this proceeding.
5. That the proposed Guardian has no interest adverse to the
minor and is not the fiduciary or an officer or employee of the corporate
! fiduciary of an estate in which the minor has an interest nor the surety
or an officer or employee of the corporate ~urety of such a fiduciary.
6. The minor is entitled to receive the sum of $30,000.00 by
virtue of the settlement of a wrongful death claim at No. 67-1098 in the
U. S. District Court for the Western Distric of Pennsylvania, which is the
sole asset of the minor's estate.

7. The minor has no claim against the United States Veterans
Administration.
8. That attached hereto is the consent of the natural parent to
this Petition anQ. to the. appointment of Western Pennsylvania National Bank,
Pittsburgh, Pennsylvania, as Guardian of the Estate of Randy S. Anselmino,
a minor.

Petition are true and correct to the best of her knowledge, information
and belief.

I, the undersigned, parent of Randy S. Anselmino, a minor,
do hereby consent to this Petition and the appointment of WESTERN
P~SYLVANIA NATIONAL BANK, Pittsburgh, Pennsylvania, as Guardian of
the estate of Randy S. Anselmino.
ACCEPTANCE OF THE PROPOSED GUARDIAN
States that WESTERN PENNSYLVANIA NATIONAL BANK being the
proposed Guardian in the foregoing Petition does hereby agree to act
as Guardian of the Estate of Randy S. Anselmino, a minor, if the
Honorable Court shall so appoint. It does hereby aver that it has no
interest which is adverse to that of the minor and further is not a
fiduciary in any estate in which the minor has an interest.
