ESTATE OF ALAN BOGUS,
a Minor.
PETITION FOR AUTHORIZATION TO PAY OVER
INSURANCE PROCEEDS TO A MINOR
TO THE HONORABLE P. VINCENT MARINO, PRESIDENT JUDGE OF SAID COURT:
The petition of ALAN BOGUS, and CHARLES R. BOGUS and
JEAN BOGUS, natural guardians of Alan Bogus, a minor, respectfully
represents:
1. That Alan Bogus is a minor who resides with
his brother, Charles R. Bogus, and the wife of Charles R. Bogus,
namely, Jean Bogus, in Centerville Borough; Washington County,
Pennsylvania, Post Office Address R. D. #2; Box 472B, Brownsville~
Pennsylvania.
2. That: the said Alan Bogus was born on June 11,
1955, and is now fifteen (15) years of age.
3. That the father of Alan Bogus, namely John C.
Bogus, died t~state 9n October 18, 1969, and by t~e provi~ions
of his Last Will and Testament, duly probated in the Register of
,. Wills Office of Washington County, .Penn~ylvania, Will Book . '
Page ___ , he devised all of his estate to his son, Alan Jean
Bogus.

4 . That a F i r s t a n d F i na l A c c o u n t wa s f i 1 e d in
the said estate, and the sum of approximately $2,800,00 in cash
will be payable to the said Alan Jean Bogus. There has been a
delay in the audit of the First and Final Account owing to an
error in the appraisal made by the Pennsylvania Transfer
Inheritance Tax Department.
5. That it has been agreed that the balance w·ill
be payable to the said Alan Jean Bogus, also known as Alan Bogus, . ' ' . I by depositing the proceeqs in a savings accow-nt':at ·the First
Federal Savings and Loan Asso~iation of Washington, Washington,
Pennsylvania; that said proceeds ,will remain in the said account

until the said Alan Bogus attains the age of twenty-one (21) years,
stibject however, to any interim order of your Honorable Court.
6. That the said Alan Bogus, also known as Alan
Jean Bogus, was the beneficiary of two ( 2) in sura nee pol ic ie s
issued by Metropolitan Life Insurance Company to the father of
Alan Bogus, namely John c. Bogus. These policies were as follows:
630 518 223-MS; 7 219 678-A. The proceeds from Policy No. 7 219
678~A amount to $273.04, and the proceeds from Policy No. 630 518
223-MS amount to $386.25.
7. That the said Metropolitan Life Insurance
Company desir~s authority to pay over the said sums to the said
Alan Bogus. A copy of the letter of the Metropolitan Life
Insurance Company to John W. Edwards, Jr.; Esq., dated January 20,
1971, is hereto attached and made a part hereof.

8. That the said Charles R. Bogus and Jean Bogus,
his w·ife, were named guardians of Alan Jean Bogus, by John c.
Bogus, in his Last Will and Testament, a copy of which is hereto
attached. That the said Charles R. Bogus and Jean Bogus join in
and consent to the payment of any proceeds due to Alan Bogus,
also known as Alan Jean Bogus, to the First Federal Savings and
Loan Association of Washington, Washington County, Pennsylvania,
under the terms and conditions hereinabove recited.
, 9. That your petitioners further request
authority to' pay the 'following expenses out of 'the proceeds of
the insurance policies:
a. 'Patrone, Cei sler, Edwards and
Pettit, attorneys' fees for preparation
of this Petition
b. Russell Marino, Clerk of Orphans t
Court, filing this P~tition and certified
copy of Order

WHEREFORE, your petitioners pray your Honorable Court
that your Honorable Court authorize and direct the Metropolitan
Life Insurance Company to pay over the proceeds of the said
insurance policies in the total sum of $659,29 to the said Alan
Bogus, and that your Honorable Court further direct that out of
the said proceeds, the sum of $50.00 shall be paid to Patrone,
Ceisler, Edwards and Pettit at attorneys' fees, and the sum of
$8.50 to Russell Marino, Clerk of Orphans' Court for the cost of
filing this Petition, and the balance to be deposited in the First
Federal Savings and Loan Association of Washington. Washington,
Pennsylvania, to be held by the said First Federal Savings and

Loan Association of Washington until the said Alan Bogus attains
the age of twenty-one (21) years, unless otherwise directed by
Order of your Honorable Court.

I PETITION FOR PAYMENT OF MONEY IN
RESTRICTED ACCOUNT TO DEVISEE AND/OR BENEFiCIARY
The petition of ALAN BOGUS, by his attorneys, PATROJ
-CEISLER, EDWARDS AND I?ETTIT, respectfully represents:
1. That, by Adjudication and Decree, dated
March 9, 1971, at No. 1288 of 1969, in the Estate of John C.
Bogus, Deceased, the sum of $2,853.51 was awarded to Alan Jean
Bogus, subject to the said sum being placed in a restricted
account at ~irst Federal Savings and~Loan Association of
Washington. ·
2. That, by Order of Court, dated March 9, 1971
at No. 63-71-248, the sum of $600.79 was awarded to Alan Bogus,
subject to the said sum being placedin a restricted account at
First Federal Savings and Loan Association of Washington.
3. That the said Alan Bogus is now 22 years of
age, having been born on June 11, 1955. An Affidavit of Age is
hereto attached and made a part hereof.
·4. That there is now the sum of $4,031.75 in
savings account #00-0128365, plus interest from March 30, 1978,
and the sum of $918.42 in savings account #00-0128609, plus
