PETITION FOR APPOINTMENT OF GUARDIAN FOR MINORS
TO THE HONORABLE JUDGES OF THE SAID COURT:
The petition of Pearl Harshey respectfully represents:
1.That Garry Lee Kirby is a minor under the age of
fourteen years having been born on July 15,1957.
2.That Madaline Irene Kirby is a minor under the age
of fourteen having been born on September 27,1959.
3.That the present residence of said minors is Box
84,Atlasburg,Smith Township,Washington County,Pennsylvania
where they reside with their mother,Pearl Harshey,your
petitioner.
4.That said minors are the children of George L.
Kirby and Pearl Harshey,your petitioner.
5.That George L.Kirby,the natural father of said
minors is deceased.
6.That appointment of a guardian for said minors

is necessary in order to process a claim under the Workmen's
Compensation Law of the State of Ohio on behalf of said minors
for the death in an industrial accident of their father,George
L.Kirby.
7.That said minors have no estates or assests other
that the possible payments to be obtained from the said claim
under the Workmen's Compensation Law.
WHEREFORE,your petitioner respectfully prays your
Honorable Court to appoint her guardian of the persons of Garry
Lee Kirby and Madaline Irene Kirby.
