PETITION FOR APPOINTMENT OF A GUARDIAN FOR A MINOR
OVER FOURTEEN YEARS OF AGE
TO THE HONORABLE,THE JUDGES OF THE SAID COURT:
AND NOW,this Ij;:rtlo day of January~,.1971,comes
GEORGETTE MITCHNER,a minor over the age of fourteen years,
by her attorney,CHRIS VLACHOS,ESQUIRE and respectfully represent
and sets forth as follows:
1.Your petitioner,Georgette Mitchner,is a
minor over fourteen years of age and resides in the Borough of
Bentleyville,Washington County,Pennsylvania at the post office
address R.D.1,Box 100,Bentleyville,Pennsylvania,15314.
2.Your petitioner was born on the 7th day of
November,1954,and is now 16 years of age.
3.Your petitioner is"the daughter of Geneva
Mitchner with whom she now resides.
4.Your petitioner's mother consents to this
petition,as is shown by her consent attached hereto.
5.Your petitioner requires a guardian for
the reason that she is to receive $8,500.00 as compensation for
injuries suffer~d in an automobile accident,said amount being
in excess of that which can be distributed to your petitioner
or your petitioner's parent.
