TO THE HONORABLE,THE JUDGE OF SAID COURT:
The Petition of June L.Sevak,by her attorney,George
K.Hanna,Esq.,respectfully represents:
1.That her name is June L.Sevak and her residence
is 306 Summit Avenue,Canonsburg,Washington County,Pennsylvan'ia.
2.That according to her birth certificate,her name
is listed as Julia Siwak,this name not being her correct name
and neither being the name her father had ever used or the name
that she had ever used.
3.She has never been known by the name of Julia Siwak;
that she has always been known by the name of June L.Sevak and
had not known her first name was Julia until she recently receive
her birth certificate on May 26,1970,which is attached hereto
and made a part hereof.
4.That due to the inconvenience on the birth certifi-
cate stating her name as Julia Siwak instead of June L.Sevak,
she is requesting that the Court issue an order having her birth
certificate corrected to her proper name,June L.Sevak,instead
of Julia Siwak.
AND she will ever pray.

AND NOW,December 1,1970,pursuant to a hearing duly held on
said date,after proper notice given,and at which all interested parties were
present and represented by counsel,this Court finds as follows:
1.That according to documentary and other substantial and
sufficient evidence presented to the Court,a female child known as Julia
Siwak,was born January 27,1911,in Canonsburg,Washington County,
Pennsylvania.It is noted that the Certification of Birth issued by the
Commonwealth of Pennsylvania,No.57099,File No.9264-11,indicates the
date of birth of this child to be February 6,1911.This is obviously an error,
as the date of filing said certificate could not be prior to the date of birth as
given.It is obvious that the dates have been reversed,the date of birth
should be given as January 27,1911,and the date of filing as February 6,
1911.
2.The name of this child was reported as Julia Siwak by a perso .
who was illiterate and not knOWledgable of the facts.The correct spelling of
the surname of said child should have been Sevak and the Christian name
should have been reported as June rather than Julia.
3.The name of the father of said child was reported as Allax
Siwak,whereas the name should have been correctly reported as Alex Sevak,
which was the proper name of said individual and which was the name under
which he had been known for many years.

4.The maiden name of the mother of said child was reported as
Hellen Namisnak,which name was correctly reported except that the Christiam
name should be spelled Helen.
5.Documentary evidence concerning these matters have been
submitted to the Court and we find them sufficient and in proper order.The
child herein concerned,June Sevak was born in Canonsburg,Pennsylvania,on
January 27,1911.We have in evidence,a deed in the year 1963,from all the
brothers and sisters of June Sevak,conveying a parcel of land to her,and the
said deed shows the names of the grantors to be Sevak and the name of the
'grantee to ge June SE?vak~,said deed is properly recorded in the Office of the

Recorder of Deeds of Washington County,Pennsylvania.We have the testimony
of M~ry Ge'orge of Canonsburg,Pennsylvania,who is aged 70 years and has.,
known June Sevak for more than 40 years;her testimony indicates that June

Sevak has been known by that name for all these years and that both her father
and her mother were also known by the name Sevak.We have the testimony of
Mary Shaw,of R.D.2,Eighty-Four,Pennsylvania,who has known June
Sevak for more than 40 years.Her testimony indicates that June Sevak has
been known by no other name and that she is still known by the name June
Sevak;that her parents both were known as Mr.and Mrs.Sevak.All of
June Sevak1s records have been carried in the name June Sevak,including her
registration in the nurses I association,she being a practical nurse and haVing
followed that occupation for many years;her registration with the Blue Cross
of Pennsylvania is and has been carried in the name June Sevak for more than
20 years.
NOW,THEREFORE,upon motion of George Hanna,Esquire,
attorney for the petitioner and under authority of Section 301 (15)of the
Orphans!Court Act of August 10,1951,P.L.1163,as amended by the Act
of August 4,1955,P.L.302,20 P.S.2080.301 (15),it is

That the Chief,Division of Vital Statistics and State Registrar of
the Department of Health of the Commonwealth of Pennsylvania,shall correct
and amend the notification of birth registration No.57099,File No.9264-11,

of ~aid Julia Siwak so that the name of said child shall be changed to June
Sevak;and the date of her birth shall be changed from February 6,1911,
to show her birth on January 27,1911;that the maiden name of the mother.,
of said child shall be listed as Helen Namisnak;that the name of the father
of said child shall be changed from Allax Siwak to Alex Sevak,and that the
fa HaWing shall be added to the registration with respect to the father:
residence,Canonsburg,Pennsylvania;co lor,white;trade,steel worker.
