PETITION FOR SETTLEMENT
OF SMALL ESTATE
TO THE HONORABLE THE PRESIDENT JUDGE OF THE SAID COURT:
The Petition of ANNA SWARTZ respectfully represents:
1.That Andrew Swartz died testate on February 7,1970,
at the Brownsville General Hospital,Brownsville,Pennsylvania,
and he was residing at 1215 Zulu Street,California Borough,
Washington County,Pennsylvania,at the time of his death.
2.That the said Andrew Swartz,by the provisions of
his Last Will and Testament,the original~of which is hereto
attached,devised and bequeathed all of his estate to his wife,
ANNA SWARTZ,your petitioner.
3.That your petition is a resident of 1215 ZUlu
Street,California Borough,Washington County,Pennsylvania.
4.That no Letters Testamentary have been granted in
the Estate of Andrew Swartz,Deceased,and there is no
contemplation of having any Letters Testamentary entered into
said Estate.
5.That the above decedent left an estate consisting
of the sum of $216.70,due to the said decedent for Black Lung
payments for the month of January,1970.

6.There are no outstanding claims against the
Estate,and all bills,including the funeral bill,have been
paid.
WHEREFORE,Your petitioner,under authority of
Sections 202 and 211 of the Fiduciaries Act of 1949,as amended
and supplemented,prays your Honorable Court to authorize and
direct the Social Security Administration to pay the sum of
$216.70 to your petitioner,ANNA SWARTZ,on account of her
family exemption.
And she will ever pray,etc.

I,ANDREW SWARTZ,of 1215 Zulu Street,California Borougl,
Washington County,Pennsylvania,being of sound mind and memory,
do hereby make,pUblish and declare this to be my Last Will and
Testament,in manner and form following,hereby revoking any
Will or Wills heretofore made by me"
FI R ST:I direct that all my just debts and funeral
expenses be fully paid and satisfied,as soon as conveniently
may be,after my decease.
SECOND:All the rest,residue and remainder of my
estate,of whatsoever type or nature,real,personal or mixed,
and wheresoever situate,I give,devise and bequeath to my wife,
ANNA SWARTZ,only in the event she survives me,and in the event
she fails to survive me,then I give,devise and bequeath all
of my said estate to my three sons,namely,ANDREW SWARTZ,JR.,
JOHN SWARTZ,and EMIL SWARTZ,to be theirs solely and absolutely
share and share alike.
THIRD.I do hereby nominate,constitute and appoint
my son,ANDREW SWARTZ,JR.,to be Executor of this my Last Will
and Testament.

hereby authorized and directed to pay the sum of $216.70 to
Anna Swartz,widow of Andrew Swartz,this sum representing the
decedent's Black Lung claim payment for the month of January,
1970.
