Woodward,Attorney for the Petitioners,it is hereby
ORDERED AND DECREED
that the parents of Richard E.Hyk,to wit,Emil Hyk and Dorothy
Hyk,his wife,are hereby appointed as guardians of said Richard
E.Hyk;and they are hereby authorized to collect from the Polish
National Alliance of the U.S.of N.A.the sum of $250.00 and
from John Hancock Mutual Life Insurance Company the sum of
$1,000.00,who are hereby authorized to pay over to said guardians
said sums,to be deposited in the Charleroi Federal Savings and
Loan Association and not to be withdrawn until the minor reaches
the age of twenty-one (21)years or upon further order of this
Court.

PETITION FOR APPOINTMENT OF GUARDIAN
TO THE HONORABLE,THE JUDGES OF SAID COURT:
The Petition of Emil Hyk and Dorothy Hyk,his wife,
respectfully represents:
1.That they are the parents of Richard E.Hyk,a minor,
who is 14 years of age,having been born on the 1st day of
October,1955.
2.That the said minor,Richard E.Hyk,is entitled to
receive the following sums from his grandfather,W1adys1aw
Popie1arski,a/k/a W1adys1aw Popirea1ski,deceased:
Polish National Alliance of the U.S.
of N.A.-Certificate No.596573
John Hancock Mutual Life Insurance Company
Insurance policy no.1188962
$250.00
1,000.00
3.That your Petitioners request that they be appointed
guardians of the Estate of the said Richard E.Hyk and that they
be authorized to collect the proceeds of the $1,000.00 insurance
policy from John Hancock Mutual Life Insurance Company and the
$250.00 Certificate No.596573 from the Polish National Alliance
of the U.S.of N.A.and deposit the same at the Charleroi
Federal Savings and Loan Association;and do hereby agree that
they will not withdraw any of the said funds belonging to the
minor without proper order of Court.
And they will ever pray.
