consideration of the within petition presented by Rose A
Two hundred eighty nine ($289.00)Dollars benefit payments under the Federal Coal
Mine Health and Safety Act of 1969 due the Estate of Louis Unicek,Deceased,is
hereby ordered and decreed to be made to Rose Anna Freed,Rudolph Unicek and
Matthew Unicek,sole surviving heirs of decedent.
And said Department of Health,Education and Welfare,Social Security
Administration,is authorized and directed to effect the distribution of said sum of
Two hundred eighty nine ($289,00)Dollars as set forth hereinabove;said distribution
to be made without the grant of any letters in the Estate of Louis Unicek,Deceased,
or further notice,and that this Decree shall be sufficient warrant for so doing,and
that,upon making such payments in distribution,the Department of Health,Education
and Welfare,Social Security Administration shall be released and discharged of and
from any further liability and accounting to anyone,for or on account of the same.

SMALL ESTATE
TO:THE HONORABLE,THE JUDGES OF SAID COURT:
The petition of Rose Anna Freed respectfully represents:
1.That your petitioner is the daughter of Louis Unicek,Deceased,
and resides in Meadowlands,Washington County,Pennsylvania.
2.That decedent,Louis Unicek,died intestate on March 20,1970 ..
a resident of Meadowlands,Washington County,Pennsylvania,being eighty-four
years of age.
3.That decedent;s entire estate at the time of his death consisted of
the following assets:
PERSONALTY:
a.Benefit payments under the Federal Coal Mine Health and Safety
Act of 1969 in the total sum of Two hundred eighty nine ($289.00)
Dollars.
REALTY:
NONE
4.That the said Louis Unicek,Deceased,was survived by the following:
heirs-at-1aw:.

a.ROZALIA UNICEK,widow of decedent,who subsequently died July
23,1970 leaving as her surViving heirs her three children,RUDOLPH UNICEK,
MATTHEW UNICEK,and ROSE ANNA FREED,who are also the sole heirs-at-law
of decedent hereinabove named.
b.ROSE ANNA FREED
c.RUDOLPH UNICEK
d.MATTHEW UNICEK
DAUGHTER
SON
SON
5.All of the creditors of decedent have been paid in full.
6.That this petition for settlement is made ?ursuant to the provisions
of Section 202 of the Fiduciaries Act of April 18,1949,P.L.512,as amended.
WHEREFORE,petitioner prays that the Court direct the Department of
Health,Education and Welfare,.social Security Administration to pay to ROSE ANNA
FREED,RUDOLPH UNICEK,and MATTHEW UNICEK the sum of Two hundred eighty
nine ($289.OO)Dollars due The Estate of Louis Unicek as benefit payment under the
Federal Coal Mine Health and S::1.fety Act of 1969.
And she will ever pray,etc.
