AND NOW,this ~ay of August,1970,comes CARMON
LA SALVIA,JR.,a minor over the age of fourteen years,by his
attorneys,PATRONO,CEISLER AND EDWARDS,and respectfully repre-
sents and sets forth as follows:
1.Your petitioner is Carmon La Salvia,Jr.,who
resides in the City of Washington,Washington County,Pennsylvania,
at the post office address of 62 Sout~Street,Washington,Pennsy1
vania.
2,Your petition~r was born on the 23rd day of
June,1952.
3.Your petitioner is the son of Carmon La Salvia
and Anna D.~a Salvia,with whom he ,now resides.
4.Your petitioner's parents consent to this
petition,as is shown by their co~sent attached hereto.
5.Your petitioner requires a guardian of his
estate for the reason that he is to receive $7315.00 as compensati n
for injuries suffered in an automobile accident which occurred
on October 18,1969 and which is the subject matter of the
complaint in trespass filed at No.223 July Term,1970,A.D.
in the Court of Common Pleas of Washington County,Pennsylvania.
6.The said amount is in excess of that which
can be distributed to your petitloner or to your petitioner's
parents.
7.Your petitio~er proposes as his guardian,
First National Bank and Trust Co.,Washington,Pennsylvania.
8.The said pro?osed guardian has no interest
adverse to your petitioner.
9.The said proposad guardian has consented
to act as guardian in writing,a~tached hereto and marked Exhibit
"A".
10.No other petition for appointment of
guardian for petitioner has been filed.
WHEREFORE,your petiti~ner,Carmon La Salvia,Jr.,a
minor over 14 years of age prays that First National Bank and
Trust Co.,Washington,Pennsylva~ia be appointed the guardian of
his estate.
