PETITION FOR APPOINTMENT OF GUARDIAN TO THE HONORABLE P.V.MARINO,JUDGE OF SAID COURT: The Petition of Anna Boothe respectfully represents: 1 ..That she is the daughter of Pauline Oleynik,also known as Pologijo Oleynik,and is a resident of the City of Balti- more,Maryland. 2.That the said Pauline Oleynik,alias pologijo Oley nik,is a widow,aged 79,and is a citizen of this state and re- sides in the Borough of Allenport,Washington County,Pennsylvania, where she has resided continuously for more than forty (40)years, and is a patient in the Brownsville Golden Age Nursing Home,5th &Church Streets,Brownsville,Pennsylvania. 3.That the said Pauline Oleynik,alias Pologijo Oley- ,nik,the alleged incompetent,has real property located in Allen- port,Washington County,Pennsylvania,amounting to appro~imately Nine Thousand Five Hundred ($9,500.00)Dollars. 4.That the said Pauline Oleynik,alias Pologijo Oley- nik,has personal property consisting of Savings Account #18 in the Roscoe Office of Western Pennsylvania National ~ank in the sum of ,, $844.76;a social Security Administration Check,dated June 3, 1970,in the sum of $98.jO;an~unliquidated personal injury claim; and miscellaneous clothing and furniture of little value. 5.That the following is a list of all persons resid- ing within and without the Commonwealth,who are sui juris,and would be entitled to share in the estate of Pauline Oleynik,alias Pologijo Oleynik,if she would die intestate: a.Mrs.Anna Boothe,daughter,1043 Wedgewood Road,Baltimore,Maryland,the petitioner. b.Mrs.Olga M.Pozum,daughter,2l2,D~nt-StreetJ Belle Vernon','Pennsylvania. c.Mr.Anthony Oleynik,son,706 East Broadwill Avenue,Albion,Michigan. d.Mr.Victor Theodore Oleynik,son;-address unknown. 6.That the alleged incompetent has no Guardian of her estate and no court has ever assumed jurisdiction in any pro- ceeding to determine her competency. 7.That the said Pauline Oleynik,alias Pologijo Oley nik,because of mental infirmities of old age,is unable to manage her property,and is liable to dissipate it or become the victim of designing persons. 8.That it is respectfully suggested that Emil A. Eckert,of Roscoe,Pennsylvania,be appointed Guardian of the estate of the said Pauline Oleynik,alias Pologijo Oleynik,should she be adjudicated to be an incompetent person;and that the said suggested Guardian has no interest adverse or hostile to the alleg- 2 .l .,,",, ed incompetent. 9.That there is attached hereto,made part of and marked Exhibit "A"the affidavit of Dr.U.A.DeRienzo,stating that he is a physician and is well acquainted with Pauline Oleynik, alias Pologijo Oleynik,and that in his opinion she is likely to become the victim of unscrupulous or designing persons,and that she could not come to Court without risk of injury to herself or others. WHEREFORE,your petitioner respectfully prays your Honorable Court to adjudicate and find that Pauline Oleynik, alias Pologijo Oleynik,is an incompetent,and to appoint Emil A. Eckert as Guardian of her estate.
