TO THE HONORABLE, THE JUDGE OF THE SAID COURT: PETITION FOR APPOINTMENT OF GUARDIAN The petition of John P. Liekar respectfully represents: 1. That petitioner, ~ phn P. Liekar is the attorney for Freda Fisher G_uratak, and maintains his office at Mellon National Bank, Canonsburg, Pennsylvania. 2. That the said Freda Fisher Guratak is 63 years of age, and is presently a patient at the Townsend Foster Home, 205 Locust Street, Washington, Washington County, Pennsylvania, having been released from Torrance State Hospital, Torrance, Pennsylvania, ::m April 8, 1970. Said alleged incompetent having been admitted to Torrance State Hospital on August 31, 1965, was released to said Townsend Foster Home on April 8, 1970, it being the opinion of the officials of Torrance that the alleged incompetent must remain in the custody of some competent person or institution. 3. That the said Freda Fisher Guratak because of mental infirmities of old age is unable to manage her property or is liable to dissipate it or become the victim of designing persons. 4. That the estate of the said Freda Fisher Guratak, so far as petitioner knows, is as follows: PERSONALTY: a. Interest in Estate of Zella M. Fisher, Deceased $3,300.00 REALTY: a. One story frame house, lots 30 1-302, situated in Chartiers Township, Washington County, Pa. 4,500.00 TOTAL PERSONALTY AND REALTY $7,800.00 .. Net income annually to the alleged incompetent from all sources known is approximately $480. 00 received from the Veterans Administration. 5. That the names and addresses of the next of kin of the alleged incompetent are: Na;mes and addresses Mrs. Edyth E. That the proposed guardian is Pittsburgh National Bank, Washington, Pennsylvania, office. 7. No other court has ever assumed jurisdiction in any proceeding to determine the competency of the said Freda Fisher Guratak. 8. That the alleged incompetent has no guardian of her estate or person. WHEREFORE, your petitioner prays that a citation issue directed to the alleged incompetent with notice thereof to the next of kin and to the superintendent of Torrance State Hospital, Torrance, Pennshlvania, to show cause why she should not be adjudged an incompetent and a guardian of her estate be appointed. .. -.. . ..
