The petition of CLARA LANCZAK respectfully represents:
J..That your petitioner is CLARA LANCZAK,an adult of
full age,'whose residence is R.D.#2,Burgettstown,Washington Cour:.ty,
Pennsylvania.
2.That the residence of the deceased was R.D.#2,
Burgettstown,Washington County,Pennsylvania.
3.That the decedent died January J.O,1970 while being a
resident of R.D.~2,Burgettstown,Wastington County,Pennsylvania.
4.That your petitioner has not applied for letters of
administration and does not intend to apply,and no other person has applied
for letters of administration to the knowledge of your petitioner.
5.That decedent left to survive him as his sole heirs-at-law
your petitioner Clara Lanczak,daughter,and Helen Rodriquez,daughter,of
3 West Pittsburgh Street,Burgettstown,'Nashington County,Pennsylvania.
6.That your petitioner maintained a family relationst~i'p with
the decedent until the time of his death.

7.That the estate of the decedent consists of a savings account
at Mellon National Bank,Burgettstown Office,in the amount of ONE HUNDRE ~
SEVENTY-FIVE and 21/100 ($175.21)DOLLARS,a premium due on policy
#FDP891 7160 from the Continental Insurance Company in the amount of
THIRTY SIX and 85/100 ($36.85)DOLLARS and a premium due on pelicy
#40-H0317239 from the Hartford Insurance Company in the amount 0:
SEVENTY-SIX ($76.00)DOLLARS.
8.That the following debts and expenses have been pc.id by
your petitioner:
Lee Funeral Home $1,123.00
9.There are no other bills and there are no creditors to the
knowledge of the petitioner.
WHEREFORE,under the provisions of Section 292 of the
Fiduciaries Act of 1949 P.,L.512.66,your petitioner requests your
Honorable Court to award the said payment of ONE HUNDRED SEVE~TY-
FIVE and 21 (:f 00 ($175.21)DOLLARS due from a savings account at Mellon
National Bank,THIRTY-SIX and 85/100 ($36.85)DOLLARS due from premiun
on policy #FDP89l7160 from the Continental Insurance Company and
SEVENTY-SIX ($76.00)DOLLARS due'from premium on policy #40-H03l7239
from the Hartford Insurance Company,to her.
