PETITION FOR THE APPOINTMENT OF A GUARDIAN FOR
THE,ESTATE OF A MINOR UNDER THE AGE OF FOURTEEN YEARS
TO THE HONORABLE,JUDGE P.V.MARINO,PRESIDENT JUDGE OF SAID
COURT:
The petition of Richard Bolton,Father of the above
named minnv,for the appointment of a guardian for his estate,
respectfully represents:
1.That Hugh D.Bolton was born April 14,1964 and
is five years of age,and resides with your petitioner and his
wife,Mr.and Mrs.Richard Bolton at 122 West Edgewood Drive,
McMurray,Washington County,Pennsylvania.
2.That said Hugh D.Bolton,a minor was injured i
an accident on 4/12/68 in Chicago,Illinois when struck by a
car operated by one Anthony Zmuda,who had liability insuranc
with Continental Insurance Casaulty Company of Chicago.
3.l'hat said Anthony Zmuda through his said insura"ce
carrier paid $700 0 for settlement of said accident case and c aim
to the duly appointed Illinois guardian of the Estate of said
Hugh D.bolton,a minor,being Paul Bolton,his uncle.
4.That said settlement was approvedJby the Probat
Division of the Circuit Court of Cook County,Illinois,and
said Paul Bolton was discharged as guardian by Order of said
Court dated February 20,1969,an exemplification of said
proceedings being attached hereto and marked Exhibit itA."
5.Under said Exhibit A the net settlement of
~6804.75 has been on deposit in the First ~ational.Bank of
Chicago,which Bank has informed Petitioner and ~~s Counsel
said money wculd be paid to a duly appointed Pennsylvania
Guardian.
6.That the within minor now has no guardian for his
estate,and has no real estate or personal property other
than said net settlement bf $6804.75 so that the apPointmen,
of a Pennsylvania guardian is necessary to receive said sum,
as said minor resides in Pennsylvania.
