PETITION FOR SETTLEMENT OF SMALL ESTATE To the Honorable,The Judges of Said Court: The petition of HELEN HUZWAY,respectfully represents: 1.That your petitioner resides at 815 Jane Street,Speers Hill,Charleroi, Washington County,Pennsylvania and is a sister of Nicholas Sine. 2.That NicholasSine,unmarried,died intestate,on January 18,1970 while a patient at the Veterans'Hospital,Pittsburgh,Pennsylvania;having his residence at 8 I ~-sA1\)'£'STi:?.\f tiT C.HAR \\£«.0 '\ / County,Pennsylvania. Washington 3.That at the time of his death the said Nicholas Sine was entitled to Social Security benefits in the amount of $272.40, 4.That at the time of his death the said Nicholas Sine left surviving him as his o~ly heirs at law the following: 'Frances Hamercheck,sister -Richeyville,Pa . .~ose ~yducek,sister -Elco,Pa. Anna Popovich,sister -Cleveland,Ohio Ma~y Densmore,sister -Cleveland,Ohio 5.That no letters of 'administration ,have been applied for nor does your petitioner intend to apply for same. 6.That there are no claims or obligations of decedent that are known to your petitioner. 7.That your petitioner assumed the responsibility and agreed to pay for his funeral services in the amount of $1,273.00 to Paul A.D'Altorio Funeral Home,71 Third Street,California,Pennsylvania. WHEREFORE,your petitioner requests your Honorable Court to authorize the proper person or persons of the Health,Education and Welfare Department of Social Security Administration to pay over to her the sum of $272.40 in benefits due decedent,Nicholas Sine,to be applied by her,:: in mitigation of funeral bill.
