TO THE HONORABLE,THE JUDGE OF SA ID COURT:
The Petition of Lyle Smith,father and natural guardian
of Diallas York,respectfully represents:
1.That Diallas York was born on January 23,1950,and
resides with her father and mother,Lyle Smith and Mar~ha Smith,
at Box IS-A,Daisytown,Pennsylvania.
2.Diallas York was married on December 29,.1968,to
John York.
3.John York was killed in an accident which occurred 0
December 20,1969.
4.There is an insurance policy payable to Diallas York
in the amount of $2,000.00 from Home Beneficial Life Insurance
Company.
5.Diallas York has incurred a funerai"bill in the
amount of $1,689.00 payable to Donald Law,135 Angle Street,
Brownsville,Pennsylvania.
6.There are no other cash assets available to Diallas
York by which to pay the funeral bill in the estate at this time.
7.There are no other creditors.

WHEREFORE,your Petitioner requests the Court to issue
a Decree directing Home Beneficial Life Insurance Company to make
direct payment to Diallas York without the necessity of appointing
a guardian or incurring further expenses.
