PETITION FOR APPOINTMENT OF A GUARDIAN
OF A MINOR UNDER AGE OF TWENTY-ONE
TO THE HONORABLE,THE JUDGE OF THE SAID COURT:
The Petition of Grace A.Squirlock,a minor under the age
of Twenty-one,by Sara M.Squirlock,her parent,respectfully
represents:
1.That Grace A.Squirlock was born June 24,1953 and is
16 years of age.
2.That the present residence of the said Grace A.
Squirlock is 12 North Main street,'Ellsworth,washington County,
pennsylvania~where she resides with her mother,Sara M.
Squirlock.'
3.The petiti~ner is the child of Sara M.Squirlock,who
resides at the above address,and Thomas P.Squirlock,now
deceased,who died on December 3,1969.
4.That the said Grace A.Squirlock is entitled to receive
the sum of $5,000.00 as her death benefit claim from ,Servicemen's
Group Life Insurance,a copy of notification of said death
benefit is attached hereto and marked exhibit "A".
5.That your petitioner does not have a guardian to whom
said distributive share can be made payable and request the
Court to appoint a suitable guardian.

WHEREFORE,your petitioner,Grace A.Squirlock,a minor
under the age of Twenty-one,by Sara M.Squirlock,her parent,
pray the court to appoint Sara M.Squirlock as Guardian of the
Estate of Grace A.Squirlock.
And they will ever pray,etc.

consideration of the annexed petition,Sara M.
appointed Guardian of the Estate of Grace A.Squirlock,a minor,
to receive and to receipt for her death benefit claim from
Servicemen's Group Life Insurance of 212 Washington Street,
Newark,New Jersey,in the sum of Five Thousand ($5,000.00)
Dollars,said amount shall be deposited by Sara M.Squirlock,
Guardian,in a restricted account in a bank covered by Federal
Deposit Insurance Corporation,said account to specify that no
part of the same,either principal or interest,may be withdrawn
without Order of this Court until said Grace A.Squirlock,
minor,becomes 21 years of age:upon initiating the account,
the bank book shall be presented to this Court for inspection.
