EXEMPTION
TO THE HONORABLE JUDGE OF SAID COURT:
The petition of Eula Mae Hart respectfully
represents:
1.That your petitioner is the widow of Loyal H.
Hart who died November 13,1969,at 4:30 o'clock P.M.,a resident
of Washington County,Pennsylvania.
2.That decedent and petitioner were married at
Langeloth,Pennsylvania,on September 19,1942 and petitioner was
member of decedent's household at the time of his death.
3.That deced.ent.died intestate leaving as his
sole heirs at law your petitioner and two children,Jacqueline
Hart,age 21 years,and Janice Hart,age 17 years,both of whom
are living at horne with petitioner at 235 East Grant Street,
Houston,Pennsylvania.
4.That decedent's funeral expenses in amount of
$1305.00 furni~hed by William Speakman Funeral Horne were paid
by your petitioner and that there are no debts against decedent's
estate.
5.That the only asset of decedent's estate which
has corne to the attention of your petitioner are the proceeds
of an endowment policy issued by The Prudential Insurance Company

$1305.00 furnished by William Speakman Funeral Home were paid
by your petitioner and that there are no debts against decedent's
estate.
5.That the only asset of decedent's estate which
has come to the attention of your petitioner are the proceeds
of an endowment policy issued by The Prudential Insurance Company

of America,being Policy No.EOS 304630 upon which there is
payable the sum of $1050.12 with interest and in which policy
the decedent had failed to designate the beneficiary of said
policy.
WHEREFORE,petitioner prays Your Honorable Court
to award to her as her family exemption and in reimbursement of
decedent's funeral expenses,the proceeds of said The Prudential
Insurance Company of America,Policy No.EOS 304630,without the
necessity of administration and appraisal.

deposes and says that the facts set forth in the annexed

AND NOW THIS ~~day of January,1970,upon
consideration of the annexed petition and upon motion of Sherman
H.Siegel,attorney for petitioner,the Court does hereby award
to Eula Mae Hart,widow of Loyal H.Hart,the proceeds of
Policy No.or Certificate No.EOS 304630 issued on decedent's
life by The Prudential Insurance Company of America as her
family exemption and to reimburse her for decedent's funeral
expenses,without administration or appraisement.
A cert~fied copy of this Decree shall constitute
sufficient authority to The Prudential Insurance Company of
America to recognize the said Eula Mae Hart as the person entitled
to receive the proceeds of said policy or certificate.
