The Petition of Norma Petrisek,by her attorney,
Michael A.Hanna,Esq.,respectfully represents:
1.That your petitioner is the widow of the deceased,
,Adam Petrisek,
2.That the residence of the decedent was R.D.#1,
Bentleyville,Washington County,Pennsylvania.
3.That the decedent died September 18,1969,at
Marianna,Washington County,Pennsylvania,and was forty-seven
(47)years of age.
4.That your petitioner and the decedent,Adam Petrisek,
lived together until the time of his death and a family relation-
ship was maintained.
5.That Letters of Administration were granted October
28,1969,appointing Norma Petrisek as Administratrix of the
jEstate of Adam Petrisek,deceased.
