A GUARDIAN FOR THE ESTATE OF A
MINOR OVER THE AGE OF FOURTEEN YEARS
TO THE HONORABLE P. V. MARINO, PRESIDENT JUDGE OF SAID COURT:
The petition of David R. Evans respectfully represents:
1. He is a minor over the age of eighteen years, having
been born on March 4, 1951.
2. Your petitioner resides at 115 Lemoyne Avenue,
Borough of East Washington, Washington County, Pennsylvania, with
his mother, Harriet Estelle Evans.
3. Your petitioner's father, G. Wesley Evans, died on
September 20, 1969, a resident of the Borough of East washington,
Washington County, Pennsylvania.

4. Your petitioner has no guardian for his estate.
5. Your petitioner's entire estate consists of the
Beneficiary on the policy of life insurance
issued in the name of G. Wesley Evans, being
Policy No. 2209008 of The Northwestern Mutual
Life Insurance Company in the face amount of
Five Thousand and No/100 ($5,000.00) Dollars,
plus accumulations in the amount of Three
Thousand Two Hundred Eighteen and 15/100
($3,218.15) Dollars, or a total of Eight
Thousand Two Hundred Eighteen and 15/100
($8,218.15) Dollars.
6. Your petitioner requests that his sister, Mary Lou
Evans, who resides at 115 Lemoyne Avenue, Borough of East wash-
ington, Washington County, Pennsylvania, be named as guardian of
his estate. She is not a fiduciary in any trust in which her
interest is adverse to that of your petitioner; and she has no
interest adverse to your petitioner and is not the fiduciary or
an officer or employee of a corporate fiduciary of an estate in
which your petitioner has an interest, nor the surety or an
officer or employee of a corporate surety of such fiduciary.
7. Your petitioner requests that the guardian be
exempted from giving bond or other security, pursuant to the Act
of April 18, 1949, P. L. 512, Section 1022 (d), 20 P. S. 320.1022
(d) and that the appointed guardian be directed to deposit the
proceeds of said insurance policy in a savings account, conditione<
that the fund may not be withdrawn until your petitioner attains
the age of twenty-one or upon prior order of your Honorable Court.
WHEREFORE, your petitioner prays your Honorable Court
to appoint Mary Lou Evans, or such other person as the Court may
determine, as guardian of his estate.

1969, upon consideration of
the within Petition for Appointment of a Guardian of the Estate
of a Minor over the Age of Fourteen Years, and upon motion of
Greenlee, Richman, Derrico & Posa, attorneys for the estate,
IT IS HEREBY ORDERED AND DECREED that Mary Lou Evans
is appointed guardian of the Estate of David R. Evans, a minor,
as prayed for; and in lieu of the entry of a bond or other
security the guardian is authorized and directed to deposit the
fund belonging to the minor, referred to in the Petition, in her
name as guardian of the Estate of David R. Evans, a minor, in

whose records shall be marked to show that no withdrawal shall be
made therefrom without Order of this Court, and the deposit book

TO THE HONORABLE P. V. MARINO, JUDGE OF SAID COURT:
The petition of Mary Lou Evans, Guardian of the
Estate of David R. Evans, a minor, respectfully represents:
1. Your petitioner was appointed guardian of the
Estate of David R. Evans, by Order of your Honorable Court
dated December 1, 1969, at the above number.
2. The only asset of the Estate was the sum of
$8,218.15 in life insurance proceeds received by the minor as
beneficiary on a policy held by his deceased father.
3. By virtue of said Order, said proceeds were
deposited in a savings account in the Mellon National Bank and
Trust Company of Washington, Pennsylvania, whose records were
marked to show that no withdrawal shall be made therefrom
without an Order of your Honorable Court.

4. The minor, David R. Evans, arrived at the age
of twenty-one (21} years on March 4, 1972, and has requested
distribution of his estate.
5. Attached hereto and designated Exhibit "A" is
the First and Final Account of your petitioner, as guardian,
duly accepted and released by said David R. Evans.
WHEREFORE, your petitioner respectfully requests
that your Honorable Court approve the guardian's first and
final Account; order that distribution be made thereunder,
after payment of the costs of this proceeding; order Mellon
National Bank and Trust Company of Washington, Pennsylvania
to permit full withdrawal of funds from savings account No.
061-5-00142; and discharge your petitioner as guardian of the
Estate of David R. Evans, a minor.

guardian from any further accounting or liability in my estate.

ORDER
upon consideration of and upon motion
of Greenlee, Richman, Posa, Attorneys for the
Estate,
IT IS HEREBY ORDERED AND DECREED, as follows:
1. That the Court does hereby approve the First
and Final Account of Mary Lou Evans, Guardian,
2. The Guardian is hereby ordered to make distri-
bution of the balance of said Estate, after payment of the
costs of this proceeding, to David R. Evans.
3. Mellon National Bank and Trust Company, Washing-
ton, Pennsylvania, is hereby ordered to permit the Guardian,
Mary Lou Evans, to make full withdrawal of the funds which she
holds in behalf of David R. Evans, a minor, in said bank's sav-
ings account No. 061-5-00142, upon presentation of an attested
copy of this Order.

making ful'l distribution in compliance with this Order, dis-
