PETITION FOR APPOINTMENT OF GUARDIAN
TO THE HONORABLE,P.V.MARINO,President Judge of said Court:
The petition of ANNA E.SHEPPARD,parent of Sherry
Sheppard,a minor,respectfully represents:
1.That your petitioner is the parent of SHERRY
SHEPPARD,a minor sixteen years of ag~who resides with your
petitioner at 308 Jones Avenue,canonsburg,Pennsylvania.
2.That the said SHERRY SHEPPARD was born on
March 3,1953.
3.That SHERRY SHEPPARD was injured as a result of
an explosion which occurred on March 11,1967,from that occur-
rence your petitioner filed suit in the Court of Common Pleas
of washington County,Pennsylvania,Civil Action-Law at
Number 290 september Term 1968 A.D.
4.That prior to trial the suit was settled and afte
bills,costs,and fees are paid the above named minor will
receive the sum of $4,500.00.
5.That the settlement referred to hereinabove was
approved by the Court of Common Pleas and by order dated
November 13,1969.
6.Your Petitioner desires that your Honorable
Court make the necessary Order to appoint Pittsburgh National
Bank as guardian of the estate of Sherry Sheppard and to allow
the guardian to receive the sum set forth above.
7.That the said Sherry Sheppard has no other estate
or property in her name except the above mentioned money.
8.That the proposed guardian has no interest adverse
to the said minor and is not the fiduciary,or an officer,or
employee,or the corporate fiduciary of any estate in which
the said minor has an interest.
WHEREFORE,your petitioner prays your Honorable Court
that Pittsburgh National Bank be appointed as guardian of the
estate of Sherry Sheppard,a minor,so that it may have and
receive the aforesaid money to be held for the said minor until
her majority.

To the Honorable P.V.Marino,Presiding JUdge of the said Court:
The Petition of PITTSBURGH NATIONAL BANK respectfully
represents:
1.That your petitioner was appointed guardian of the
above named minor on November 26,1969.
2.That the said minor was born March 3,1953 and is
now 18 years of age.
3.That the guardian account held by your petitioner
is made up of proceeds from a recovery made on behalf of the
said minor from injuries received by him arising out of a civil
claim.
4.That your petitioner has in its guardian account
investments at a cost of $4373.00 and univested cash in the
amount of $6.50.
5.That your petitioner has received a statement
from Dr.H.Neiman that the said minor is in need of replacement
for upper missing teeth and the cost for the services to be
rendered by Dr.Neiman is $337.00.A copy of the said statement
is attached hereto and made a part hereof.

6.That the said minor is now married and that her
husband is unemployed and without funds to make payment for the
necessary dental services.
Wherefore your petitioner prays your Honorable court
to authorize and direct it to pay the sum of $337.00 to Dr.H.
Neiman for dental services to be rendered by Dr.Neiman to the
said minor,Sherry Sheppard.

the costs for the clothing and necessary medical services to be
obtained for rerself and her child Gabrielle Moore is $250.00.
6.That the said minor is now married and that her
husband is unemployed and without funds to make payment for the
necessary clothing and medical services.
WHEREFORE your petitioner prays your Honorable Court
to authorize and direct it to pay the sum of $250.00 to the
minor Sherry Sheppard Moore for clothing and medical expenses
