PETITION FOR THE APPOINTMENT OF A GUARDIAN
OF THE ESTATE OF A MINOR OVER THE AGE OF
FOURTEEN YEARS
TO THE HONORABLE, THE PRESIDENT JUDGE OF SAID COURT:
The Petition of Sharon Michaux respectfully represents:
1. That she is a minor having been born on June 1, 1952, and that she
resides with Mary Michaux, her natural mother and Leonard A. Michaux, her
natural father, at Box 469, Daisytown, Washington County, Pennsylvania.
2. That your petitioner has no guardian of her estate.
3. That the Estate of the minor is as follows:
(a) The beneficiary under Metropolitan Life Insurance.Company
Policy N>. 15500 Gin the amount of $7,500. 00, Gerald
Michaux insured, brother of the Petitioner,. now deceased as
of August 6, 1969.
4. It is necessary for the appointment of a guardian of the estate of
the minor to protect her interests and prevent her from becoming the victum of
designing persons.

5. Your petitioner proposes that _Western Pennsylvania National Bank
be appointed guardian of h~r estate.
6. The proposed guardian has no. interest adverse to the minor, and
is not a fiduciary or officer or employee of any corporate fiduciary of an estate
in which the minor has an interest, nor the surety nor an officer or employee
of the corporate surety of such a fiduciary.
7. Tha:t the proposed guardian has no interest in any matter which is
adverse to or in conflict with the interests of the petitioner.
WHEREFORE, your petitioner prays that Western Pennsylvania
National Bank be appointed guardian of the estate of Sharon Michaux.

to the best of her knowledge, information and belief.

We agree to accept the appointment as Guardian of the Estate of Sharon
