PETITION FOR APPOINTMENT OF GUARDIAN
TO: THE HONORABLE, THE PRESIDENT JUDGE OF SAID COURT:
The petition of Harry F. Wheeler and Louise E. Wheeler
respectfully represents:
1. That your petitioners, Harry F. Wheeler and Louise
E. Wheeler, his wife, are the parents of Janet Wheeler, who is
17 years of age, having been born on June 12, 1952, and who
resides with the said Harry F. Wheeler and Louise E. Wheeler
at 940 Arch Street, Washington, Washington County, Pennsylvania.
2. That no guardian has ever been appointed for the
said Janet Wheeler, minor daughter of your petitioners, and
a necessity arises for the appointment of a guardian for her
Estate by reason of the pending settlement of a trespass action
for personal injuries suffered by the said Janet Wheeler, a
minor, as a result of an automobile accident on the lOth day of
May, 1969 on Route 18 near the Washington County Airport.
3. That as a result of the pending settlement of the
said trespass action for personal injuries, there will come into

the ha~ds of the guardian to be appointed for the said minor,
the sum of $5,569.41.
4. That your petitioners make choice of, and
respectfully request your Honorable Court to appoint the First
National Bank & Trust Company of Washington, Pennsylvania, Beau
and Jefferson Avenues, Washington, Pennsylvania guardian of the
Estate of Janet Wheeler, a minor. There is attached hereto a
consent of the said First National Bank & Trust Company to act
as such guardian, and consent of the said minor, Janet Wheeler,
to the appointment of such guardian.
5. That the proposed guardian, First National Bank
& Trust Company of Washington, Pennsylvania, has no interest
adverse to Janet Wheeler, a minor.
WHEREFORE, your petitioners make choice of, and
pray your Honorable Court to appoint the First National Bank
& Trust Company of Washington, Pennsylvania guardian of the
Estate of Janet Wheeler, a minor.
LOUISE E. WHEELER, Petitioner.
CONSENT

sylvania hereby agrees to act as guardian of the within minor's
Estate, if the court should appoint it as such guardian.

PETITION FOR TERMINATION OF GUARDIAN
TQ: THE HONORABLE, THE PRESIDENT JUDGE OF SAID CCURT:
AND NOW, comes JANET WHEELER, now married and known as
JANET KADEMENOS and sets forth the following:
1. That your Petitioner Janet Wheeler, now married and known as
Janet Kademenos is an adult individual and resides at 940 Arch Street,
Washington, Pennsylvania. On October 16, 1969, your Petitioner was 17
years of age and received a settlement in the amount of $5569. 41 in a Trespass
Action for Personal Injury suffered by your Petitioner as a result of an
automobile accident which occurred on the 1Oth. day of May, 1969.
2. On or about the aforementioned date, Harry F. Wheeler, and
Louise E. Wheeler, his wife, and Parents· of the Petitioner filed with this
Court a Petition for the Appointment of a Guardian for said Petitioner.
On or about the aforementioned date, the Court approved the appointment of
First National Bank and Trust Company of Washington, located on West Beau
Street, Washington, Pennsylvania 15301 as guardian of the Estate of Janet
Wheeler, a minor, but now married and known as Janet Kademenos.
3. On or about October 16, 1969, the funds received in the settlement
of said Trespass Action in the amount of $5569. 41 were deposited in the First
National Bank and Trust Company in Washington as guardian of Janet Wheeler

a minor, now married and known as Janet Kademenos, savings account having
Account Number 06-07542-8-20.
4. On June 12, 1973, Petitioner Janet Wheeler, now married and
known as Janet Kademenos, attained the age of 21 years of age; proof being
her birth certificate, a copy of which is attached hereto and marked as
Exhibit "A".
WHEREAS, Janet Wheeler, now married and known as Janet Kademenos
has reached her majority. It is hereby requested that the Honorable Court
terminate the First National Bank and Trust Company of Washington, Pa.
as the Guardian of the said Janet Wheeler, now married and known as Janet
Kademenos and any remaining funds in said account be turned over to her.

from the foregoing Petition, that Janet Wheeler, n married and known as
Janet Kademenos, has attained the age of 21 years, it is hereby ordered and
decreed that the First National Bank and Trust Company of Washington,
guardian of the Estate of Janet Wheeler, married and now known as Janet
Kademenos, hereby be terminated and the remaining balance in said Estate
Savings Account Number 06-07542-8-20 be turned over to Janet Wheeler,
now married and known as Janet Kademenos.
