PETITION
The Petition of EMMA ROSE TENNY respectfully represents:
1. That she is the mother of Cindy Jane Dubina, and
resides at 205 Orchard Avenue, Canonsburg, Washington County,
Pennsylvania.
2. Said child is presently known as Cindy Jane Tenny
and the name of the father on the birth record is not listed, a
copy of said Birth Certificate attached hereto as Exhibit A.
3. On April 22, 1962, the date of the birth of said
child, the name and address of the unmarried natural mother was
Emma Rose Dubina, 354 Vercheck Drive I Houston, Washington co·.lnty I
Pennsylvania.
4. On March 26, 1966 your Petitioner was married to
Charles A. Tenny, the natural father, a copy of the marriage
certificate is attached hereto as Exhibit B.
5. No property rights will be adversly affected by
the granting of this Petition.
WHEREFORE, your Petitioner prays that the birth record
of Cindy Jane Dubina be amended to read as follows:
(a) NAME OF CHILD: Cindy Jane Tenny
(b) NAME OF FATHER: Charles A. Tenny
(c) ADDRESS OF FATHER: 354 Vercheck Drive,
Houston, Washington County, Pennsylvania.
(d) COLOR: White
(e) AGE OF FATHER: 17
(f) BIRTH PLACE OF FATHER: Waynesburg, Greene
County, Pennsylvania.

l. That·Charles A. Tenny and Emma Rose Tenny, his wife
are the natural parents of Cindy Jane Dubina, a minor, seven (7)
years of age, and
2. That a·t the time of the birth of said minor on
April 22, 1962 the said parents were unmarried, but were sub-
sequently married on March 26, 1966, and
3. That the said parents are presently husband and
~ife and support said minor who forms part of their household.
NOW, THEREFORE, upon motion of CHRIS VLACHOS, Esquire,
attorney for petitioners, and under authority of Section 301
(15) of the Orphans• Court Act of 1951, P.L. 1163, as amended by
the Act of August 4, 1955, Act No. 116, it is
ORDERED AND DECREED
That the Chief, Division of Vital Statistics, and State
Registrar of the Department of Health of the Commonwealth of
Pennsylvania shall correct and amend birth registration No.8989
so that the name of the child, Cindy ·Jane Dubina, shall be changed
to Cindy Jane Tenny, that the name of the father shall be recorded
as Charles A. Tenny and that the following shall be added to the
registration with respect to said father:
(a) AGE: 17
(b) COLOR: White
(c) BIRTH PLACE: Waynesburg, Greene County,
Pennsylvania
