PETITION
TO THE HONORABLE p.V.MARINO,
PRESIDENT JUDGE OF SAID COURT:
The Petition of Maurice Herz1ich respectfully represents:
1.That he is the son-in-law of Samuel Krasik,a/k/a
Sam Krasik,and resides at 122 Prospect Avenue,Charleroi,
Washington County,Pennsylvania.
2.That Samuel Krasik,a/k/a Sam Krasik,formerly
resided at 122 Prospect Avenue,Charleroi,Washington County,
Pennsylvania,but by virtue of his illness is now in the Charleroi
Monessen Hospital,North Charleroi,Pennsylvania.
3.That in the interest and well-being of Samuel
Krasik,a/k/a Sam Krasik,the appointment of a guardian of his
person is essential for the following reasons:
(a)The said Samuel Krasik,a/k/a Sam Krasik,is
87 years of age,having been born on October
12,1882.
(b)The assets of Samuel Krasik,a/k/a Sam Krasik,
consist of the following:

1.Real estate situate in the Borough of
Charleroi,Washington County,Pennsylvania
120-132 Lincoln Avenue,$50,000.00
Charleroi,Pennsylvania
upon which there is erected
a two-story brick commercial
'building on whic~there are
three monthly leases totalling
$625.00 per month
122 Prospect Avenue,11,500.00
Charleroi,Pennsylvania,
upon which there is erected
a two-story frame dwelling
2.The personal estate of Samuel
Krasik,a/k/a Sam Krasik,
consists of:
1200 shares of A.T.&.T.common
stock
200 shares of International
Harvester.common':stock
600 shares of Gulf Oil common
stock
Charleroi Federal Savings and
Loan Association savings account
in the amount of $13,000.00
Mellon National Bank and Trust
Company certificate of deposit
in the amount of $25,000.00
Miscellaneous U.S.Savings Bonds
approximating $15,000.00
Checking account at Mellon National
Bank and Trust Company in an
indeterminate amount
4.The said Samuel Krasik,a/k/a Sam Krasik,has
recently undergone a loss of mentality,has been incapable of
keeping records and is otherwise incapable of taking care of his
financial affairs.
5.The only person who has any interest in the we11-

being of Samuel Krasik,a/k/a Sam Krasik,is his daughter,Ruth
Herzlich,whose husband is the Petitioner herein.
6.That Samuel Krasik,a/k/a Sam Krasik,has no
guardian of his person.
7.That your Petitioner respectfully requests he be
appointed guardian of the person of Samuel Krasik,a/k/a Sam
Krasik,to take care of his affairs and to collect the rents,
issues and profits from his various assets or to cause or allow
the same to be so done because of his familiarity with the
financial condition of the alleged incompetent or,in the
alternative,your Petitioner prays your Honorable Court to
appoint a national bank to do and perform those things which the
said Samuel Krasik,a/k/a Sam Krasik,is incapable of performing,
with the same force.and effect as if he could personally do the
same.
WHEREFORE,your Petitioner prays that the Court
appoint a guardian for the person of Samuel Krasik,a/k/a Sam
Krasik.

DeRienzo,M.D.,who,being duly sworn according to law,deposes
and says that he has been the family doctor for Samuel Krasik,a/k/
Sam Krasik,for many years;that he has been in treatment of him
for the condition of arteriosclerosis and related illnesses for
some period of time;that the physical condition of the said
Samuel Krasik,a/k/a Sam 'Krasik,has resulted in a mental conditio
of advanced senility whereby the said Samuel Krasik,a/k/a Sam
Krasik is no longer capable or responsible for the care and
preservation of his assets;that he is incapable of performing
usual business functions insofar as having any accurate or comp1et
knowledge of his assets,'costs,expenses,income and other factors
usually common knowledge to one who has real estate and other
income-producing properties;that by virtue of the nature of the
condition of Samuel Krasik,a/k/a Sam Krasik,it is believed
that the best interest and welfare of the physical condition of
this person would be best served by having appointed a guardian
to do and perform those things which the said Samuel Krasik,
a/k/a Sam Krasik,can no longer do of his own free will and
volition;that Samuel Krasik,a/k/a Sam Krasik,is incapable
of appearing in person in court for any hearing for the reason
that he is·bedfast and confined to the Charleroi-Monessen Hospital
