TO THE HONORABLE P.V.MARINO,PRESIDENT JUDGE OF SAID COURT:

1.The name of your petitioner is Joseph Kamp,whose
mailing address is Box 96,Lawrence,Washington County,Pennsyl-
vania.
2.The names of the minors are Mark Schultz and Bernard
Schultz,whose ages are 10 and 11,respectively.
3.'The said minors presently reside with their parents,
Mr.and Mrs.Donald P.SChLItz,at 35 Ridge Avenue,Canonsburg,
Washington County,Pennsylvania.
4.That your petitioner is the step-grandfather of
said minors.
5.That it is necessary that a guardian be appointed
for the above named minors ~ecause they are beneficiaries of a
certain insurance policy (M~tropolitan Life Insurance CompanYL
arising under the estate of Leonard'P.Schultz,deceased.
6.That no guardian has been appointed for said minors
previously.
7.The proposed guardian has no interest adverse to
the minors and is no~the fiduciary or an officer or employee of

any corporate fiduciary oian ·'estate in which the minors,have an

8.That Metropolitan Life Insurance Company will not
distribute any funds to.tte minor beneficiaries'herein until and
unless a guardian is appointed for said minors as more fully set..'.forth in a letter from sai1 Company,which is 'hereto attached,
made a part hereof and marked Exhibit "A".
9.The minors'estate consists of the following assets
and has the value as indicated below:
a)Mark Schultz .$4,000.00
b)Bernard Sch~lt~. . . . . .4,000.00
WHEREFORE,your petitioner requests the Court,pursuant
to Section 1011 of the Fiduciaries Act of April 18,1949,P.L.,
512,20 P.S.320.1011,to appoint Joseph Kamp as guardian for
the esta~es of said minors.

This 1s to advise you that s1nce the remainder ot the proceeds under
tl11s Group pollcy are payable to two minor beneficiaries,Bernard and
Mark Schultz,we are in need of guardianship papers.Please advise us
if these guardianship papers have been issued and 1f not,please supply
us the reason for the delay.
We are also enclosing Forms 10451 and if no guardian has been or will be
appointed,these proceeds may be held to accumulate interest unt11 age 21.
If th1s is desired,please have these enclosed 10451 forms ccmp1eted and
returned to us.
