The Petition of RICHARD A.PHILLIPS,Administrator D.B.N.respectful y
represents:

That your petitioner is the father of Donald W.Phillips,Deceased,
and together with Florence J.Phillips,the decedent's mother,are the sole
heirs of the decedent.

That the decedent,Donald W.Phillips,died as a result of an
accident on DeCember 9,1967,and the sole asset in his estate was the right
to recover under the survival action which suit was filed in the Court of
Common Pleas of Washington County,Pennsylvania,at No.231 November Term,
1968,A.D.

That as a result of said action,your petitioner did recover for
the estate the sum of $1,991.00.

That your petitioner has p~id the sum of $1303.77 for funeral
expenses to the William D.Rogers Funeral Home of McDonald,Pennsylvania,and

there are no other debts or creditors of the decedent of which your petitioner

That no wages have been paid directly to members of decedent's famil
under Section 201 of the Act of 1949 and none are anticipated,as the decedent
was not employed for a long time prior to his death.

That this Petition for Settlement is made pursuant to the provisions
of Section 202,of the Fiduciaries Act of April 18,1949,P.1.512,as amended
(20 P.S.320:202:)and Section 211 of the same Act;as amended (20 P.S.
320:211:)•
WHEREFORE,your petitioner prays your Honorable Court to enter a
Decree of Distribution awarding the sum of $1303.77 unto your petitioner for
repayment of the funeral expenses;and the balance of $687.23 to your petition r
and Florence J.Phillips;his wife,as the sole heirs at law.
