ADJUDICATION AND DECREE
And now September r,19-.b9-,this matter came on for hearing,
audit and distribution at this session and testimony taken;and thereupon,upon due consideration
thereof the balance for distribution in the hands of the Accountant is determined to be
$530.21 and the account is accordingly confirmed;and it is ordered,
adjudged and decreed that the said balance be paid out by the Accountant in accordance with the
schedule of distribution hereto attached and made a port hereof,unless exceptions hereto be filed

KNOW ALL MEN BY THESE PRESENTS,That I,ELEANOR JUDY WORK,
having attained the age of twenty-one years,do hereby acknowledge that
I have this day received of and from Pittsburgh National Bank,my guardian
duly appointed by the Orphans'Court of Washington County,Pennsylvania,
the sum of Five Hundred Nineteen and 71/100 ($519.71)Dollars,being the
amount awarded me at No.820 of 1969 of the Orphans'Court of Washington
County,Pennsylvania,in full satisfaction and payment of all claims which
I have,or might have,against my said guardian,and of all monies or
property for which it may be accountable,and I do,therefore,by these
presents,remise,release,quit-claim and forever d~scharge the said
Pittsburgh National Bank,and its successors,of and from the said guardian-
ship,and of and from all actions,suits,payments,reckonings,claims and
demands whatsoever,for or by reason thereof.
