PETITION FOR APPOINTMENT OF GUARDIAN
TO THE HONORABLE, THE JUDGE OF SAID COURT:
The Petition of MILDRED KUREK respectfully represents:
1. That your petitioner, Mildred Kurek, is a resident
of R. D. #1, Scenery Hill, Washington County, Pennsylvania.
2. That the alleged incompetent is Rose Fulton, who is
years of age, having been born on the seventy-six (76)
5th day of April · .1893 , and has been a resident of
R. D. #1, Scenery Hill, Washington County, Pennsylvania, and is
now a patient at Washington Hospital.
3. That the alleged incompetent, Rose Fulton, has been
in the Washington Hospital since June 3, 1969, because of mental
incompetency and mental illness. She is unable to manage her
property and is liable to dissipate it or become a victim of
designing persons.
4. The following are the only persons who would be
entitled to a share in the estate of the above named alleged
incompetent if she dies intestate at this time:

Scenery Hill, Penna.
Frances Boles
103 Mayfair Lane
Tonawanda, New York
Charles Fulton
R. D. #1
Scenery Hill, Penna.
Hazel Danley
R. D. #3
Washington, Penna.
5. The total estate of the alleged incompetent consists
of a home and real estate situate in North Bethlehem Township,
R. D. #1, Scenery Hill, Washington County, Pennsylvania; that
the alleged incompetent receives the sum of One Hundred Thirteen
and 90/100 ($113.90) Dollars monthly social security benefit.
6. That the alleged incompetent is not a member of the
armed services.
7. That the petitioner is the daughter of the alleged
incompetent and has no adverse interests to the alleged incompeten •
8. That no other Court has ever assumed jurisdiction nor
has there been any proceeding to determine competency of the
alleged incompetent and no guardian has heretofore been appointed
for said alleged incompetent in this or in any other jurisdiction.
WHEREFORE, the petitioner prays that Rose Fulton be
adjudged an incompetent and that she be appointed Guardian in the
Estate of Rose Fulton.
