PETITION FOR AUTHORITY TO RECEIVE MONIES ON BEHALF OF A MINOR
TO THE HONORABLE PATSY V.MARINO,PRESIDENT JUDGE OF SAID COURT:
The petition of Anne J.Cornell respectfully represents:
1.That she is the natural guardian of Richard Scott
Cornell,a minor.The said minor is the child of the petitioner
and the deceased Donald S.Cornell,who died December 9,1968,
and the said minor resides with your petitioner at 224 Union Stre t,
california,washington County,Pennyslvania.
2.That the said minor was born on March 20,1953.
3.That the estate of the said minor consists of being
named co-beneficiary for his deceased father's Teachers'Retireme t
System,in the State of Maryland.The benefits for such retirem nt
system is in the amount of a~proximately One Thousand,Two Hundre
Fifty and no/IOO ($1,250.00)Dollars.
4.That no guardian of the estate of the said minor
has been appo4nted.
WHEREFORE,your petitioner requests that the sum that
the said minor is entitled to from the Teachers'Retirement Syste ,
in the State of Maryland,be paid to Anne J.Cornell,as natural
guardian of the said minor.
