PETITION OF MOTHER OF MINOR TO RECEIVE
".PROCEEDS OF INSURANCE POLICY
AND NOW,this 15th day of May,1969,comes LOIS
JEAN CONNORS,by her attorneys,Patrono,Ceisler and Edwards,
Esquires'r and respectfully represents and sets forth to your
Honorable Court as follows:
1.Your Petitioner is a widow who resides at
,'l4'1.Sharon Avenue,Washington,Pennsylvania.
2.Your Petitioner is the widow of Richard J.
Connors who met his death in an automobile accident on June
9,1968.
3.The estate of the said Richard J.Connors
is.before Your Honorable Court,partially administered,and
it appears that the said estate will be insolvent counting as
a cla~m against the estate your petitioner's claim for a 'family'
exemption.
4.Your Petitioner is the mother of Carol
Lee Connors,who was born on December 1,1950.
5.The said Carol Lee Connors resides with
your petitioner in her household and is a member of your
petitioner's family.Your petitioner cares for and maintains
the said Carol Lee Connors.
6.On March 1,1959 the Protective Life
Insurance Company,of Birmingham,Alabama,issued its policy
of life insurance No.179550 on the lives of Richard J.Connors

and your petitioner and the survivor in which the said Carol Lee
Connors was beneficiary.The said policy provides for a benefit
of $1,000.00 to be paid to the said Carol Lee Connors on or
after March 1,1969.The benefits under the said policy,$1,000.a
are now due.and payable to Carol ~ee Connors but she is a minor.
The Protective Life Insurance Company has,as a consequence
of her minority,refused to pay the said sum until it receives
appropriate directions from Your Honorable-Court.
7.Other than her interest in the apparently
insolvent estate of Richard J.Connors 'the said Carol Lee
Connors owns no property of any kind,_other than the proceeds in
the said insurance policy.
WHEREFORE,Your Petitioner requests that your
Honorable Court direct Protective Life Insurance Company to pay
the proceeds of the said insurance pOlicy-to your petitioner
as mother of the said minor who maintains the said minor,without
the appointment of a guardian or the entry of security,_to be used
for educqtional purposes

ordered that it be filed and it is ORDERED AND DECREED that Lois
Jean Connors,mother of Carol Lee Connors,a minor,by whom the
said minor is maintained in her household,is authorized entitled,
and directed,under the provisions of the Pennsylvania Fiduciaries
Act of April 18,1949,P.O.512.1001,as.amended,20 P.S.320.1001
to receive the proceeds of Policy No.179550 issued by protective
Life Insurance Company to Richard James Connors and Lois Jean
Connors,of which Carol Lee Connors is beneficiary;and it is
further ORDERED AND DECREED that the said Protective Life InsurancE
Company pay the proceeds of the said policy to the said Lois Jean
Connors without any appointment of a guardian or the entry of
security.The fund is to be placed in a restricted account in the
,First Federal Savings and Loan Association of Washington and with-

ORDER
AND NOW I April 5,1972,upon cause shown,the First
Federal Savings and Loan Association of Washington,is authorized and
directed to pay the full balance in the Account of Carol Lee Connors,a
minor;this Court is informed that the balance standing to this minorIs
credit is approximately $1,000 at present.
An order was made by this Court on May 19,1969,directing

the funds of the above minor to be placed in a savings account in the First
Federal Savings and Loan Association of Washington,with directions to pay
the full balance of said account to the minor upon her arriving at the age of
21 years.This Court is satisfied that the former minor,Carol Lee Connors,
arrived at the age of 21 years on December 1,1971.The said Association
is hereby authorized and directed to pay the full balance in said account to
the said former minor,upon a proper order signed by the said former minor
Carol Lee Connors,and by her mother and natural guardian Lois Jean
Connors.Upon said payment the said Association shall be discharged of all
.liability in connection with the withdrawal of the full balance of said fund as
indicated.
