PETITION FOR THE SETTLEMENT OF A SMALL ESTATE AND FOR A FAMILY
EXEMPTION AS PROVIDED BY THE FIDUCIARIES ACT OF 1949 AS AMENDED,
SECTIONS 202 AND 211 RESPECTIVELY
TO THE HONORABLE,P.VINCENT MARINO,PRESIDENT JUDGE:
The Petition of Mary Buc.sok,a/k/a Mary Buchak,respect-
fully represents:
1.That your petitioner is Mary Bucsok,a/k/a Mary
Buchak,of the Borough of Donora,Washington County,Pennsylvania.
2.That the decedent,George Bucsok,a/k/a George
Buchak,died on January 29,1969,at the Memorial Hospital in
New Eagle,Washington County,Pennsylvania,and who at the time of
his death was a resident of 637 Sixth Street,Donora,Washington
County,Pennsylvania.
3.That the petitioner is the surviving spouse of the
decedent and was a member of his household at the time of his
death;they were married on May 15,1915,at St.Michael's Greek
Catholic Church,Donora,Washington County,Pennsylvania.
4.That the decedent died intestate,and no Letters have
been applied for or granted by the Register of Wills of Washington
County,Pennsylvania.
5.The names and relationship of those interested in
this estate as next of kin are:
Mary Bucsok,a/k/a Mary Buchak,surviving spouse
Ann Churan,daughter,Monessen,Pennsylvania
John Buchak,son,Charleroi,Pennsylvania
Mary Camber,daughter,Donora,Pennsylvania
Helen Moffett,daughter,Charlotte,North Carolina
George Buchak,son,Cleveland,Ohio
Tommy Buchak,son,Donora,Pennsylvania
Albert Buchak,son,Charleroi,Pennsylvania
Joseph Conrad Buchak,son,Phillipine Islands
Eileen Smith,daughter,Rochester,New York
6.The person entitled to the Family Exemption as pro-
vided by Section 211 of the Fiduciaries Act of 1949 as amended is
Mary Buc.sok,a/k/a Mary Buchak.
7.The property of the decedent and the value of the
same is as follows:

8.The following disbursement has been made by the
surviving spouse on behalf of the estate prior to the filing of
this Petition:

9.The petitioner knows of no unpaid claimants of this
estate.

distribution of the property to Mary Bucsok,a/k/a Mary Buchak,as
reimbursement for the monies advanced as set forth in Paragraph 8

Allor any transfer agents to whom the above-described property ar
presented for transfer are to give full effect to this Order and
are relieved of any liability by the transferring of said property
to Mary Bucsok,a/k/a Mary Buchak.
