This was recorded in Birth Docket Vol.3 page 294,Item 5. 2.The Petitioner alleges that this date is inaccurate and that the proper date of his birth was April 19,1904. 3.The birth certificate issued by the Register of Wills Office of Washington County,Pennsylvania,is attached hereto as Exhibit "A"show- ing the data taken from the records of that office. 4.The Petitioner in support of his claim that he was born on April 19,1904,has attached a Certificate of Baptism of the Methodist Church of Monongahela,Pennsylvania,signed by George H.Giles,the IPresent minister of that church. 5.The Petitioner in further support of his claim as to the accural~e klate of his birth will submit for the Court's inspection a life insurance lPolicy issued on May 12,1930 by the Connecticut Mutual Life Insurance k;ompany of Hartford,Connecticut being policy No.708682 wherein the J:>irth of the Petitioner is indicated as April 19,1904. 6.In further support of his claim as to the accurate date of his birth the Petitioner will make available to the hearing Judge a little booklet bearing the name of "C.E.Beach"with entries made from time to time ...• as to the family history and genealogy.This book contains the following entry "Chauncey Beach Yohe born April 19,1904 @ 4:00 A.M.II It is sub- mitted that this is an entry normally made in the family Bible and in manYi respects this booklet can be treated the same way as far as containing re- cords of births,marriages and deaths of this family. 7.The Petitioner submits the additional following information in the event it might be important.The Petitioner is a white male and his birth was legitimate and single.His father's name was Clyde Calvin Yohe who died on July 28,1942 and his mother's name was Nellie Beach Yohe who died on June 13,1953.His mother's maiden name was Nellie Cordeli Beach.The Petitioner's mother and father i.e.Nellie Cordelia Beach and Clyde Calvin Yohe were married on July 2,1903. 8.The Petitioner is now retired.His former occupation was as an Engineer with the U.S.Army Corp of Engineers. 9.The Petitioner's address is 4870 Chriss Road,Bethel Park, Allegheny County,Pennsylvania. WHEREFORE the Petitioner reque st your Honorable Court to make an Order directing that the records in the Register of Wills Office of Was - ington County,Pennsylvania,be corrected to reflect the accurate date birth. ORPHANS'COURT DIVISION IN RE: Amendment of Birth Record of CHAUNCEY BEACH YOHE ( ) ( ) ( No.1Mof 1969 ORDER DIRECTING CORRECTION OF BIRTH RECORD AND NOW,AprilS,1969,it appears to the Court that an error was made in the entry of the record of birth of Chauncey Beach Yohe,who was born in Monongahela,Washington County,Pennsylvania. Proper evidence of his date of birth has been submitted to the Court to-wit,a certificate of baptismal record issued by the First Methodist Church at Monongahela,Pennsylvania,which shows the birth of said Chauncey Beach Yohe to have been on April 19,1904,and the said baptismal record is attached hereto and marked Exhibit liB II;also a photocopy of a page of a small booklet showing family history and genealogy;the said page contains the following entry:IIChauncey Beach Yohe born April 19,1904,at 4:00 A.M.'I.The said photocopy is attached hereto and marked Exhibit IIC II and the Court recognizes said entry as one normally made in the Family Bible and in many respects can be treated in the same way as a Bible,as far as containing records of births,marriages,and deaths of this family. There is further submitted to the Court a photocopy of a life insurance policy issued May 12,1930,by the Connecticut Mutual Life Insurance Company of Hartford,Connecticut,wherein the birth of the applicant,Chauncey Beach Yohe,is indicated as April 19,1904.Said photocopy is attached hereto and marked Exhibit IID II . NOW,THEREFORE,IT IS ORDERED AND DECREED that the record of the birth of said Chauncey Beach Yohe recorded in the Office of the Register of Wills of Washington County,Pennsylvania,in Birth Record or Volume No.3,Page 294,Item.s:shall be corrected to show the birth of said Chauncey Beach Yo he to be on April 19,1904;that all other matters recorded in said birth record shall remain as heretofore; that a notation shall be entered in said Docket,in red ink,showing the authority of the Clerk to make said correction by citing this Order . .......
