To the Honorable P.Vincent Marino,President Judge of said Court.
The Petition of Thomas T.Chopak respectfully represents
1.That Peter Chopak was 79 years of age and his
permanent address was 8 Giffin Drive,Canonsburg,Washington
County,Pennsylvania.
2.That the said Peter Chopak died on November 22,1968
while living at the above mentioned address.
3.That your Petitioner is Thomas T.Chopak,who
resides at 8 Giffin Drive,Canonsburg,Washington County,Pennsy1-
vania,and is a nephew of the decedent but who did not form part
of the household at the date of the 1atters death.
4.That the Heirs of the decedent are as follows,all
of whom are sui juris:

5.That the decedent was unmarried during his lifetime.
6.That Peter Chopak died leaving a will dated March
20,1968,filed in the Office of Register of Wills of Washington
County,Pennsylvania.A copy of said will is attached hereto and
made a part hereof and marked Exhibit HA u •
7.That Letters Testamentary were issued to your
Petitioner,who is named as Executor in said will,on March 21,
1969.
8.That under the terms of said will the decedent's
entire estate was bequeathed to your petitioner,Thomas T.Chopak.
9.That under dates of March 27,April 3,10, 17,24,
and May 1,1969,your Petitioner duly advertised the Grant of
Letters.A copy of the Affidavit and Proofs of Publication are
attached hereto and made a part hereof and marked Exhibit tlB".
10.That the property of the decedent at the time of
his death consisted of the following:
