PETITION FOR APPOINTMENT OF GUARDIAN
TO THE HONORABLE,THE JUDGE OF SAID COURT:
The petition of Noreen Frances Kubincanek,mother and
natural guardian of James H.Vicheck,Jr.,a Minor,Susan Vicheck,
a Minor,Janyce Vicheck,a Minor,and Stephen Vicheck,a Minor,
respectfully represents:
1.That James H.Vicheck,Jr.,is a minor born
Ma'y 31,1957.
2.That Susan Vicheck is a minor born December 15,195 '.
3.That Janyce Vicheck is a minor born OCtober 28,
1962.

4.That Stephen Vicheck is a minor born December 11,
1964.
5.That the present address of the minors is Box 315,
Avella,Washington County,Pennsylvania.That James H.Vicheck,
Jr.,Susan Vicheck,Janyce Vicheck and Stephen Vicheck,minors,
reside with their mother and natural guardian,Noreen Frances
Kubincanek.
6.The petitioner is the mother and natural guardian
of James H.Vicheck,Jr.,Susan Vicheck,Janyce Vicheck and
Stephen Vicheck.
7.That James H.Vicheck,Jr.,Susan Vicheck,Janyce
Vicheck ~nd Stephen Vicheck are the children of Noreen Frances
Kubincanek,who resides at the above address,and James H.Vichec ,
Sr.,who died August 19,1965.
8.That your petitioner has no interest in the
proceeding.
9.That the Mellon National Bank and Trust Company,th
suggested guardian,is neither executor,administrator,nor
trustee in any estate in which the minors have an interest nor
surety for any such fiduciary,and has no interest adverse so
that of the minors whatsoever.
10.That the estate of James H.Vicheck,Jr.,consists
of the sum of $3,333.00,the estate of Susan Vicheck consists of
the sum of $3,333.00,the estate of Janyce consists of the sum
of $3,333.00 and the estate of Stephen Vicheck consists of the
sum of $3,333.00 and compensation benefits to be paid by the Stat
Workmens Insurance Fund of the Commonwealth of Pennsylvania in
accordance with Compensation'Agreement No.7 ,_955,948 as provided
by the Act,the same being the amount which will be paid to their
guardian in settlement and compromise of a suit entered against
The Duquesne Light Company,a corporation,and Acme Welding
Company,a corporation,and Clarence Lutch &Son,Inc.,a
corporation,Civil Action No.66-747 in the United States Distric
Court for the Western District of Pennsylvania,on account of in-
juries sustained by James H.Vicheck,Sr.,which resulted in his
death due to the negligence of the defendants,the said James H.
Vicheck,Sr.,was the husband of Noreen Frances KUbincanek,your
petitioner herein,and the father of James H.Vicheck,Jr.,Susan
Vicheck,Janyce Vicheck and Stephen Vicheck,minors herein.
WHEREFORE,your petitioner,Noreen Frances KUbincanek,
mother and natural guardian of James H.Vicheck,Jr.,Susan
Vicheck,Janyce Vicheck and Stephen Vicheck,minors,prays the
Court to appoint the Mellon National Bank and Trust Company,
a corporation,guardian of their estates.

Gentlemen:
Janyce has the opportunity to participate in a supervised,
educational trip to France this summer with her French instructor
and other students from her high school.The total cost of the trip is
estimated to be $1500.00.
I am requesting a disbursement from Janyce's guardianship funds
to meet the cost of this trip.Please inform me if the distursement
can be made from the funds.Your immediate reply will be appreciated,
as the deadline for the money to be paid to the sponsoring group is
the middle of April.
