The Petition of JOHN W.EALY,by his attorney,John
Solomon,Esq.,respectfully represents:
1.That your petitioner is the son of Essie J.Ealy
a/k/a Essie Ealy,deceased.
2.That the residence of the decedent was R.D.#1,
Dunna Station,Washington County,Pennsylvania.
3.That the decedent died February 11,1969,at
Washington,Washington County,Pennsylvania,and was eighty-two
(~2)years of age at the time of her death.
4.That your petitioner has not applied for Letters of
Adr'ini~tration and does not intend to apply for Letters,and no
other person has applied for Letters of Administration to the
knowl~dge of your petitioner.
5.That the estate of the decedent is composed entirely
of personal property consisting of a check from Pennsylvania Blue
Shield,Check No.6886454,in the amount of $149.20 made payable
to Essie Ealy.
6.That the decedent left to survive her,the following
heir:
