PETITION FOR APPOINTMENT
OF GUARDIAN
TO THE HONORABLE,THE PRESIDENT JUDGE OF SAID COURT:
The petition of Joellen Brodak,a minor,and Joseph R.
Brodak,her parent,respectfully sets forth:
1.The name of the minor is Joellen Brodak.She was
born September 24,1954,and is fourteen (14)years of age.She
and her parent above named reside in Washington County,Pennsyl-
vania,their post office address being R.D.#3,Washington,
Pennsylvania 15301.
2.The minor's parent consents to and joins in this
petition.
3~The appearance of the minor in Court would interfere
with her schooling,and such appearance is not necessary for the
reason that there are no complications or difficulties in con-
nection with this petition.
4.Appointment of a guardian for said minor is
necessary and the minor has no guardian.
5.The proposed guardian is Union National Bank of
Pittsburgh.
6.The proposed guardian has no interest adverse to the
minor,and is not the fiduciary or an officer or employee of the
corporate fiduciary of an estate in which the minor has an interes ,
nor the surety or an officer or employee of the corporate surety
of such a fiduciary.
7.The estate of the minor consists of the sum of
Four Thousand One Hundred Ninety ($4,190.00)Dollars in cash,
being the proceeds of a settlement for personal injuries suffered
by the minor in an accident,which settlement has been duly ap-
proved by the Court of Common Pleas of washington County,Pennsyl
vania,in accordance with the provisions of the Pennsylvania Rule
of Civil Procedure.
8.No insurance or other gratuity is payable to the
minor by the United States veterans'Administration or its
successor.
WHEREFORE,petitianer prays tha t the Court will appoint
Union National Bank as guardian of the estate of Joel1en Brod.ak,
a minor.
