DOLLARS UNDER ART.II,SEC.202 OF THE FIDUCIARIES
ACT OF 1949,P.L.512,66,AS.AMENDED
The Petition of ANNA LIPSCOMB,respectfully represents:
1.That your petitioner is the widow of Richard W.
Lipscomb,Deceased.
2.That the residence of the decedent was 524 Short
Street,Washington,Washington County,Pennsylvania.
3.That the decedent died December 19,1968,at Washington,
Washington County,Pennsylvania,and was fifty-seven (57)years
of age.A family relationship was maintained between your
petitioner and the deceased until the time of his death.
4.That your petitioner has not applied for Letters of
Administration and does not intend to apply for letters and no
other person has applied for Letters of Administration to the.
knowledge of your petitioner.
5.That the estate of the decedent is composed entirely
of persona.l property,being proceeds of personal injury settle-
ment of the decedent vs.Suburban Lines,Inc.,and Herbert:J.
Wise,in the amount of $2,250.00,arising out of an accident which
occurred at Washington,Pennsylvania,February 20,1968,
6.That the decedent left to survive him,the following
heirs:

9.That this petition for settlement is made pursuant
to the provisions of 'Art.II.Sec.202 of the Fiduciaries Act of
1949,P.L.512,as amended,as Richard W.Lipscomb,deceased,
domiciled in the Commonwealth of Pennsylvania,in the County of
Washington,and leaving an estate composed of cash of a gross
value not exceeding Twenty-five Hundred ($2,500.00)Dollars.
10.That this Petition is also a Petition for Family
Exemption requesting that any balance of the money after payment
is made to the H.D.Burch Mortuary be paid to her as her family
exemption.
WHEREFORE,your petitioner prays your Honorable Court
to enter a Decree of Distribution of the proceeds of personal
injury settlement of the decedent vs.Suburban Lines,Inc.,and
Herbert J.Wise,in the amount of $2,250.00 without further notice
or with such notice as your Honorable Court may deem necess~ry and
without grant of letters testa~entary.

and upon motion of John Solomon,Esq.,Attorney for the Petitioner,
it is ORDERED,ADJUDGED AND DECREED that Anna Lipscomb,Widow of
Richard W.Lipscomb,deceased,be and is hereby authorized to
execute complete release and that proceeds of personal injury
settlement of the decedent vs.Supurban Lines,Inc.,and Herbert
J.Wise,in the amount of $2,250.00,be distributed in the
following manner:
TO:Anna Lipscomb,the sum of Two Thousand Two Hundred Fifty
and no/100 ($2,250.00)Dollars for payment of funeral expense in
the amount of Seven Hundred Fifty-four and no/100 ($754.00)
Dollars;Dr.Wilson B.Pizzi,for services rendered in connection
with said personal injury claim,Seven Hundred and no/100 ($700.00)
Dollars;to Washington Hospital,Thirty-six and.no/100 ($36.00)
Dollars;to John Solomon,Attorney,one-third contingent fee
Seven Hundred Fifty and no/100 ($750.00)Dollars;for filing
Petition,Ten and no/100 ($10.00)Dollars,and the Security
Insurance Group,insurance carrier for Suburban Lines,Inc.,and
Herbert J.Wise,is directed and authorized to issue draft payable
to Anna Lipscomb and John Solomon,her attorney,in the amount of
Two Thousand Two Hundred Fifty and no/100 ($2,250.00)Dollars
to be distributed as decreed herein.'
