PETITION FOR APPOINTMENT OF GUARDIAN
TO THE HONORABLE, P. V. MARINO,
President Judge of said Court:
The petition of HANNAH MAE HARRISON, parent of EARLEEN
HARRISON, a minor, respectfully represents:
1. That your petitioner is the mother and sole parent
of Earleen Harrison, a minor, 20 years of age who resides with
her at R. D. #1, Pleasant View Road, Finleyville, washington
County, Pennsylvania.
2. That the said Earleen Harrison was born September
25, 1949.
3. That your petitioner desires that a guardian be
appointed for Earleen Harrison because she is to receive as
beneficiary, the sum of $4,875.00 from the Nationwide Insurance
Company on account of the death of your petitioner' s husband
and the father of the minor child, Edward Francis Harrison.
4. That the said sum is an agreed amount that is to be
paid to the said minor under a Court Order settling the accident
case which took the life of the said Edward Francis Harrison on
May 27, 1967.
5. That the said minor had no other property or estate
of her own except the sum above mentioned.
6. That your petitioner respectfully requests that
Pittsburgh National Bank, Washington Office, be appointed
guardian of the minor's estate.
7. That the proposed guardian has no interest adverse
to the said minor and is not the fiduciary or an officer or
employee or the corporate fiduciary of any estate in which the
said minor has an interest, nor the surety or an officer or
employee of any corporate surety of such a fiduciary.
WHEREFORE, your petitioner prays that the said
PITTSBURGH NATIONAL BANK, Washington Office, be appointed as
guardian so that they may have and receive the aforesaid money
and any income mentioned in the said petition.

attained the age of twenty-one years, do hereby acknowledge that I have
this day received of and from PITTSBURGH NATIONAL BANK, my guardian,
duly appointed by the Orphans' Court of Washington County, Pennsylvania,
the sum of FOUR THOUSAND EIGHT HUNDRED FIFTEEN and 13/100 ($4,815.13)
DOLLARS, being the amount awarded me at No. 70 of 1969 of the Court
of Common Pleas of Washington County, Pennsylvania, Orphans' Court Division,
in full satisfaction and payment of all claims which I have, or might have,
against my said guardian, and of all monies or property for which it may
be accountable, and I do, therefore, by these presents, remise, release,
quit-claim, and forever discharge the said PITTSBURGH NATIONAL BANK, and
its successors, of and from the said guardianship, and of and from all
actions, suits, payments, reckonings, claims, and demands whatsoever, for
