II, SEC. 202 OF THE FIDUCIARIES ACT OF 1949, P. L. 512, 66, AS AMENDED The Petition of Elizabeth Sturchak respectfully represents: 1. That your petitioner is the widow of Charles Sturchak, deceased. 2. That the residence of the decedent was 753 Mary Street, Bentleyville, Washington County, Pennsylvania. 3. That the decedent died November lH, 1968, at 753 Mary Street, Bentleyville, Washington County, Pennsylvania, and was seventy-two (72) years of age. A family relationship was maintained between your petitioner and the deceased until the time of his death. 4. That your petitioner has not applied for Letters of Administration and does not intend to apply for letters and no other person has. applied for Letters of Admin~tration to the knowledge of your petitioner. 5. That the estate of the decedent is composed entirely of the proceeds from an endowment with the John Hancock Mutual Life Insurance Company in the amount of $1,123:26 plus interest in the approximate amount of $43.17. 6. That the decedent left to survive him, the following heirs: NAME AGE RELATIONSHIP RESIDENCE Elizabeth Sturchak Sui Juris Widow 753 Mary Street Bentleyville, Pa. Daniel Charles Sturchak Sui Juris Son Bentleyville Pennsylvania And furthermore, there are no other heirs who may have and assert a claim for the share of the estate of the decedent. 7. That your petitioner is sui juris. 8. That the only creditor of the estate of the decedent is as follows: NAME PLACE OF BUSINESS Kenneth V. Thompson Funeral Home Bentleyville, Penna. AMOUNT $975.00 9. That this petition for settlement is made pursuant to the provisions of Art. II. Sec. 202 of the Fiduciaries Act of 1949, P. L. 512, as amended, as Charles Sturchak, deceased, domiciled in the Commonwealth of Pennsylvania, in theCounty of Washington, and leaving an estate composed of the proceeds from an endowment with the John Hancock Mutual Life Insurance Company of a gross value not exceeding Twenty-five Hundred ($2,500.00) Dollars. 1'0. That this Petition is also a Petition for Family Exemption requesting that the balance of the money after payment is made to the Kenneth V. Thompson Funeral Home be paid to her as her family exemption.
