PETITION FOR APPOINTMENT OF GUARDIAN
TO THE HONORABLE,P.V.MARINO,President Judge of said Court:
The petition of ALVIE C.JACOBS and ELIZABETH J.
JACOBS,parents of DAVID LEE JACOBS,a minor,respectfully
represents:
1.That your petitioners are the parents of DAVID
LEE JACOBS,a minor,13 years of age who resides with his
parents at R.D.#4,McDonald,Cecil Township,Washington
County,Pennsylvania.
2.That the said DAVID LEE JACOBS was born on
November 14,1955.
3.That your petitioners desire that a guardian
be appointed for DAVID LEE JACOBS because he is to receive,
as beneficiary,the sum of $20,000.00 from Prudential Life
Insurance Company on account of the death of his brother
1st Lt.Donald W.Jacobs.
4.That the said 1st Lt.Donald W.Jacobs was in-
sured by Prudential Insurance Company of American under policy
no.35 294 594 which sum became due and payable to the named
beneficiary,the minor herein,upon the death of 1st Lt.Jacobs
while serving in Vietnam on June 11,1968.
5.That the said minor has no other property or
estate of his own except the sum above mentioned.
6.That your petitioners respectfully suggest that
PITTSBURGH NA.TIONAL BANK,Washington office,be al?pointed
guardian of the minor's estate.
7.That the proposed guardian has no interest
adverse to the said minor and is not the fiduciary or an
officer or employee or the corporate fiduciary of any estate
in which the said minor has an interest,nor the surety or an
officer or employee of any corporate surety of such a fiduciary.
WHEREFORE,your petitioners pray that the said
PITTSBURGH NATIONAL BANK,Washington office,be appointed as
guardian so that they may have and receive the aforesaid money and
any income mentioned in the said:.ipetition.
