PETITION FOR APPOINTMENT OF A GUARDIAN
TO THE HONORABLE, THE PRESIDENT JUDGE OF SAID COURT:
COMES NOW Lila Johnson, and petitions this Honorable Court
as follows:
1. That Petitioner, Lila Johnson, is the natural mother of
the above-named minors.
2. That said minors reside with their mother at 575 Allison
Avenue, Washington, Washington County, Pennsylvania.
3. That the birth dates of said minors are as follo~s:
Jay Richard Johnson -September 22, 1955
Judy Lynn Johnson -September 2J, 1963
That attached hereto, marked Exhibits 11A11 and "B", and made a part
hereof, are certified copies of the birth certificates of said minors.
4. That James R. Johnson, father of said minors, died on
December 10, 1967, as a result of injuries received in an automobile
accident which occurred on December 7, 1967, in the City o~ Allentown,
Lehigh County, Pennsylvania.
5. That said James R. Johnson died intestate. On January 25,
1968, the Register of Wills of Lehigh County, Pennsylvania, granted
Letters of Administration on the Estate of James R. Johnson, Deceased,
(File No. 39-1968-88) to Gladys Hoare, 1121 Donamy Glen, Seater. Plains,
New Jersey.

6. That a lawsuit has been instituted in the United States
District Court for the Eastern District of Pennsylvania as follows:
Gladys Moore, Administratrix of the Estate of James R. Johnso~, Deceased,
vs. Twin County Trans-Video, Inc., Civil Action No. 68-442.
7. That the lawsuit has been settled for the gross amount of
Fifty Thousand ($50,000.00) Dollars, out of which each of the minors
will receive Nine Thousand ($9,000.00) Dollars, directly from the insurance
carrier of defendant. An additional sum of Two Thousand ($2,~00.00)
Dollars will be distributed from the Administratrix of the Estate after
all estate expenses are deducted.
8. That Petitioner proposes The Pittsburgh National Bank to
act as guardian of the Estate of each of said minor children. Attached
hereto, marked Exhibit "C", and made a part hereof, is a letter dated
July 17, 1968, signed by Charles E. Enlow, Trust Officer of the Bank,
consenting to act as Guardian of the Estates of said minors.
9. That the proposed guardian has no interest adverse to the
minors and is not the fiduciary or an officer or employee of the corporate
fiduciary of an estate in which the minors have an interest nor the surety
or an officer or employee of the corporate surety of such a fiduciary.
WHEREFORE, Petitioner prays that The Pittsburgh National Bank,
Washington, Pennsylvania, be appointed guardian of the Estate of said
minors.
