TO THE HONORABLE PRESIDENT JUDGE OF SAID COURT:
The petition of Harriett C.Davis,respectfully represents:
1.That Lola Long 7 a resident of the City of Was~ington,.
Washington County,Pennsylvania',died at the age of.fifty-seven
(57)yea~s on June 2,1968,leaving'her Will dated January 16,
1968,wherein she devised and bequeathed her eritire estate unto
your petitioner.A true'anc.correct copy 'of.said Will is attachec
hereto and made a part hereof.
2.The'only.asset cf the Estate of said decedent-was
the'Social Security burial benefit payable to :the~'decedent who
paid the burial expense of her husband,Ralph C.Long,who died
January 13,1968.
3.Decedent because of the condition of her health was
unable to collect said Social Security burial benefit during her
lifetime.
4.That your petitioner as sole beneficiary under the
Will of decedent,is entitled to payment of·said Social Security
burial benefit.
5.That your'petitioner has paid the burial expenses of

the decedent in amount of $1060.00 to Robert E.Hummell Funeral
Home.and has no knowledge of any unpaid creditors of decedent.
WHEREFOP~,petitioner·prays your Honorable Court

Personally appeared before me,the undersigned
authority,HARRIETT C.DAVIS,who being duly sworn according to
law deposes and says that she .is the informant with regard to the
estate of Ralph C.Long who died January 13,1968:that she was
a niece by marriage to the decedent;that by-deed dated April 1,
1966,which was twenty-two months prior to his death,the said
Ralph C.Long executed a deed conveying the property described in
the annexed Inheritance Tax Return to himself and his wife,Lola
,Long,as tenants by the entireties;that at the time of the
execution of said deed,to wit:April 1,1966,the said Ralph C.
Long was 58 years old;that he had been afflicted with pulmonary
emphysema but that he was in fairly good health and had a life
expectancy of several yep.rs i .that on or about the day before
his death,decedent \\Tas,stricken with a coronary thrombosis and
died within 24 hours;that this affidavit is-given for the purpose
of overcom~ng the presumption that said deed was made by decedent
to himself and wife in contemplation of death.
