AND NOW,September 6,1968,it appears to the Court that an
error was made in the entry of the record of birth of Walter McCrerey who
was born in Buffalo ToW'nship,ViTashington County,Pennsylvania,in that the
surname of the said individual was improperly listed as "McCrearyl'.
Proper evidence of the correct surname of said individual has beer
submitted to the Court,to wit,the identification card from the Social Security
AdministratiDn showin~Social Security No.162-16-8416 to have been issued tc
one Walter Ivan McCrerey;the said Social Security card was issued to him
many years ago at the time ;)f the initial application of said individual for
Social Secur~ty listing;the identification card from the Keystone Insurance

said individual during :he period when Aircraft Warning Service was being
