AND NOW~comes NORBERT LECHWAR,1 and petitions your
Honorable Court as follows:
1.That Donna Kopac is a minor residing at R.D.#2"
Canonsburg..Washington County,Pennsylvania.
2.That'Donna Kopac is a minor twenty (20)years of age"
having been born on January 22,1948.
3..That on or about July 6...1966,the said minor was involved
in an accident in which she received serious injuries"which injuries have
been permanent and the said minor is still not able to perform her normal
functions~
4.That the said .minor has a cause of action against the per'"
son,t and persons"who negligently caused her to be so injured.
5.Thafyour Petitioner is a cousin of the said minor child"
who resides at Meridan Street,t Morgantown..West Virginia.
6.That your Petitioner intends to file,on behalf of the minor
a civil action in the U.S.District Court for the Western District of Pennsylvan a
to recover damages which he believes are due the minor on account of the
negligence involved.

7.That your Petitioner requests appointment for the purposes
of bringing said suit and upon recovery it is the Petitioner's intention to re'"
quest discharge in favor of a banking institution if such should become neces'"
sary at the time of the litigation.
WHEREFORE,t Petitioner prays your Honorable Court to
appoint him Guardian of the Estate of Donna Kopac"a minor"for the purpose
of bringing whatever action may be necessary to recover on her behalf for the
injuries received as a result of the negligence of a third party.
