RECEIPT AND RELEASE
KNOW ALL MEN BY THESE PRESENTS,That I,WILLIAM J.KOZIEL,JR.,
having attained the age of twenty-one years,do hereby acknowledge that
I have this day received of and from Pittsburgh National Bank,my guardian,
duly appointed by the Orphans'Court of Washington County,Pennsylvania,the
sum of Four Thousand Three Hundred Eighty-nine and 16/100 ($4,389.16)Dollars,
being the amount awarded me at No.829 of 1968 of the Orphans'Court of
Washington County,Pennsylvania,in full satisfaction and payment of all
claims which I have,or might have,against my said guardian,and of all monies
or property for which it may be accountable,and I do,therefore,by these
presents,remise,release,quit-claim and forever discharge the said Pittsburgh
National Bank,and its successors,of and from the said guardianship,and of
and from all actions,suits,payments,reckonings,claims and demands whatso-
ever,for or by reason thereof.
